Provide a "Certificate of Liability Insurance" naming Interfaith Family Shelter, as additional <br /> insured to protect Interfaith Family Shelter, against the risks covered by Cascade View <br /> Presbyterian Church insurance policy; <br /> 10. Arrange towing of unauthorized and abandoned vehicles at owner's risk and expense; <br /> 11. Identify a contact person(s) who will be available to ensure that communication <br /> between organizations will be accurate and timely; <br /> 12. Meet with interfaith representatives at least once a month; <br /> 13. Communicate with Interfaith promptly when the following events occur: <br /> a. an incident, <br /> b. a grievance <br /> c. client leaves the program, <br /> d. 911 call related to Program. <br /> SECTION II. <br /> Substantial form of the Code of Conduct provisions: <br /> 1. Participation the Cars to Housing (C2H) program requires background checks for each <br /> individual age 13 and older, and participants are subject to, and agree to, searches of person <br /> and property while on the church grounds. <br /> 2. Personal information will be kept confidential within the Interfaith Family Shelter Staff. <br /> Demographic information will be released to CVPC staff. <br /> 3. You will be given a designated location to park and sleep. <br /> 4. You will have a permit giving you an assigned space for up to 90 days. This permit may be <br /> renewed if space is available on a day-by-day basis. <br /> 5. Only one vehicle is permitted per family. All family members must sleep inside the vehicle. <br /> 6. A portable toilet and trash bins will be available in the parking lot. Trash must be placed in <br /> the appropriate bin (garbage, or paper recycling). The toilet is serviced once a week but may <br /> be serviced more depending on site population. <br /> 7. Cleanliness of the site and the toilet will be all guests' responsibility. <br /> 8. Smoking is permitted in the parking lot. Cigarette butts must be collected and disposed of <br /> properly. <br /> 9. Each car must openly display the required Cars to Housing Permit Form. Cars left unattended <br /> in the parking lot for 48 hours will be subject to being towed at the owner's expense. <br /> 10. Tents, canopies, and lean-tos are not permitted.The use of lawn chairs is permitted, <br /> however they must be kept clean and put away when not in use. All personal items must be <br /> taken with you when you depart the site. <br /> 11 <br />
