EVERETT <br /> WASHINGTON <br /> City of Everett Human Needs Grant <br /> January 1, 2019 through December 31, 2019 <br /> Exhibit A <br /> Program Summary <br /> Organization& Program: Interfaith Association of Northwest Washington: Cars to Housing <br /> SECTION L <br /> Interfaith Family Shelter ("Interfaith"), a Washington non-profit agency, and Cascade View <br /> Presbyterian Church ("CVPC") have the general understanding with respect to the services provided <br /> by Interfaith Family Shelter, and the parking spaces, services, facility modifications, and upkeep of <br /> church grounds offered by Cascade View Presbyterian Church at 1030 E. Casino Rd, Everett, WA <br /> 98203. The parking spaces will be used for up to five homeless families and/or individuals living in <br /> their vehicles. This MOU is intended to outline the basic responsibilities of the parties that each will <br /> attempt to carry out in good faith for the Cars to Housing Program (Program). <br /> In order to carry out the Program outlined above the parties agree to the following roles and <br /> responsibilities: <br /> Interfaith Family Shelter shall: <br /> 1. Provide overall supervision and administration of the Carto Housing Program located at the <br /> CVPC facility; <br /> 2. Develop and provide an efficient process for referrals, screening, intakes, and on-going <br /> case management for up to five families or individual women living in their cars in CVPC's <br /> parking lot; <br /> 3. Work with CVPC to update the Program Code of Conduct for those staying in CVPC's <br /> parking lot. The Code of Conduct shall include the provisions shown in Section II of <br /> Exhibit B. Interfaith shall submit a copy of any changes to the Code of Conduct within 30 <br /> days following implementing the new provisions. <br /> 4. Set up a phone line(s) for potential and current clients to call; Respond to current clients <br /> within two hours or an aftemative timeframe agreed upon by both parties; <br /> 5. Accept eligible clients who qualify for the Program; <br /> 6. Request that clients fill out an Informed Consent for Release of Information (ROI) to <br /> appropriate staff at Interfaith Family Shelter and CVPC for the purpose of coordination of <br /> care; CVPC shall be consulted in the drafting of such an ROI and shall have editing authority; <br /> 7 <br />
