NOTICE OF APPLICATION <br />AND <br />PROPJSED MITIGATED DETERMINATION OF NON-SIGNIFICANt;t <br />SEPA # 07-048 <br />Schmidt Medical — Office Building <br />October 19, 2007 <br />Description of Proposal: Construction of a new two (2) story 5,000 square foot medical office <br />building and associated site improvements in a B-2 zone. <br />Applicant: Schmidt Investment Group <br />1217 128"' Street NE <br />Marysville, WA 98271 <br />Location: 4418 Rucker Avenue <br />Zoning: B-2, Community Business <br />General Plan: 4.4 Mixed Use Commercial — Multiple Family <br />Lead Agency: City of Everett Planning Department <br />Contact Person: Faheem Siddiq Phone: (425) 257-7142 <br />Mitigation Measures: <br />The environmental impa tc of this proposal are documented in the Environmental Checklist and <br />other information on file with the City. The listed requirements are placed in response to our <br />review of this information: <br />AGENCIES WITH JURISDICTION <br />None <br />MITIGATION OF ADVERSE IMPACTS BY SEPA <br />1) Any grading/fill on the site shall be done so as to not impact the surrounding properties. <br />(SEPA Earth, Land and Shoreline Use Policies). <br />2) Prior to issuance of any permits, the applicant must pay a mitigation fee of $10,575.00 per <br />the Interim Traffic Mitigation Ordinance No. 1670-89, as amended. (SEPA Transportation <br />Policies). <br />The lead agency for this proposal has determined that it does not have a probable significant <br />adverse impact on the environment. An Environmental Impact Statement is not required under <br />RCW 43.21 C.030(2)c. This determination assumes compliance with State law and City <br />ordinances related to general environmental protection including but not limited to right-of-way <br />improvement requirements, drainage, etc. This decision was made after review of a completed <br />
