Second iloor structural calculations required for permit B0901-019 for "fl - NEW SECON... Page 1 of 4

Meg Haley
From: Meg Hal=y
Sent: Monday, May 11, 2009 5:08 PM
To: 'gdp@gte.neY
Cc: 'Harvest Christ;an Center InPI'
Subject: RE: Second floor structural calculations required for permit 80901-019 for TI - NEW SECOND
FLOOR OFFICES at 830 SE EVERETT MAL WAY
Rth,
Sorry it took a while to get back to you on this. You now have the inspection reports for the two permits that have
not been issued: sanctuary and halcony plus the second floor officea. You also should have received the
inspection report for the religious rooms and nursery, permit number 80801-047, which was issued on 10123/08.
In general, work has proceeded on all three permits without matching up to the pians as a whole. Specifically:
. The structural information or, all three permits is either incomplete or inaccurate as compared to the as-
built condition. No building inspections were scheduled for the issued permit B0801-047. Work has
proceeded on permits that have not yet been issued: 80901-018 and -019.
. The architectural information on the permits for the religious rooms and second floor were not built as
shown on the plans, primariiy around the former elevator location and out from that towards the street. The
guards and handrails on both outside exits do not meet code.
. There is a new interior window in the nursery that is not shown on the plans and is located in a bearing
wall, and will require a header, if the window is to remain in that locat!on.
. The sanctuary will need addifional support at the balcony, and the supporting structure for the balcony
(beams and columns) also require a tested one-hour fire resistance rating.
. We agreed (the Pastor, you and I) that the structural drawings and calclations should be revised,
collected and submitted as complete and accurate for the permit currently issued, B0801-047, since the
one hour fire resistant fleor ceiling assembly cannot be installed untii we have reviewed and stamped the
structural framing plans and calculations.
• The one hour floor ceiling assembly should also be submitted for thi BJa01-047, since the floor is already
constructed. The calculations should take into account the loading of the floor above (dead and live) and
the construction (one hour fire resistive), and the roof if the structural support of that has been altered.
• The storage wili need to be removed from the second floor corridor overlooking all of the ciassrooms and
sanctuary; otherwise, an elevator will be required to the second floor.
The building inspectors are going out tomorrow for a consuitation inspection with Pastor Kulakevich and Valerie to
discuss how to get the project back on track as far as the permit and inspection process. As I understand it, you
will also be going out tomorrow to review the as-built condition for the three permits, and revising drawings to
meet code, where the as-built condition varies from the architectural plans suhmitted for permit or issued for
permit.
That sums up what happened in the field last week. Let me know how you would like to proceed, once you have
a chance to get out to the site tomorrow.
My best to your husband and your mom.
-Meg
From: Ruth Gonzales [mailto:gdp@gte.net]
Sent: Wednesday, May 06, 2009 3:03 PM
To: Meg Haley
Subject: RE: Second floor structural calculations required for permit 80901-019 for TI - NEW SECOND FLOOR
OFFICES at 830 SE EVEREfT MALL WAY
5/ 1 I /2009
