Washington State STOP Formula Grant Program Renewal Application for FFY 2019 <br /> Coordinated Community Response (CCR)Team Participation <br /> A condition of receiving these funds is mandatory attendance at the local Coordinated Community <br /> Response team meetings or tribal Coordinated Community Response team meetings, which must meet at <br /> least quarterly during the grant period. <br /> Renewal Application Budget <br /> The budget for this continuation of STOP Grant activities must correspond to your currently approved <br /> budget. Goods and services must be proportionate to the grant-funded staff activities and must be direct <br /> project costs. Round budget numbers to the nearest dollar. <br /> Reporting Requirements <br /> One report will be required covering the period January 1, 2020 through December 31, 2020. Grantees <br /> must submit the report data electronically using the report form provided by OCVA. Grantees using InfoNet <br /> will not be required to use the report form provided by OCVA unless the InfoNet data is incomplete. <br /> Grantees shall maintain documentation and records that support the data reported.Annual progress report <br /> forms will be due to OCVA with the final invoice, but not later than 30 days after the end of the grant <br /> period. <br /> STOP Certification Requirements for Law Enforcement and Prosecutors <br /> Criminal justice applicants must certify their jurisdiction complies with the statutory eligibility requirements <br /> of the STOP Grant Program. Criminal justice applicants unable to certify compliance with the statutory <br /> eligibility requirements of the STOP Grant Program will be prohibited from receiving these funds. <br /> Equal Employment Opportunity Program <br /> Funding for the STOP Grant Formula Grant Program is subject to the Equal Employment Opportunity <br /> Program requirements. If you have questions regarding the Equal Employment Opportunity Program <br /> requirements, please visit the DOJ Office for Civil Rights website at www.ojp.gov/about/ocr/eeop for more <br /> information. <br /> EEO Certification Form <br /> Your grant document will include the Certification Form for Compliance with the Equal Employment <br /> Opportunity Program Requirements. <br /> Suspension or Termination of Funding <br /> Grant funding may be suspended in whole or in part,terminated, or other sanctions imposed on a recipient <br /> for the following reasons: <br /> • Failing to comply substantially with the requirements or statutory objectives of the Violence Against <br /> Women Act, program guidelines issued thereunder, or other provisions of Federal law. <br /> • Failing to make satisfactory progress toward the goals, objectives, or strategies set forth in the <br /> application. <br /> • Failing to adhere to the requirements in the grant terms and conditions. <br /> • Proposing or implementing substantial plan changes to the extent that, if originally submitted, the <br /> application would not have been selected for funding. <br /> • Failing to submit reports. <br /> 4 <br />
