location. The component parts of such a billing shall not be segregated and placed on separate billing forms for the purpose of sending same to more than one location. Section 65: Charges, Notices and Communications - Where Sent. All water charges, special charges, delinquent notices and other notices and communications shall be sent to the address of the premises served except as hereinafter provided. If the owner, or his authorized agent for the premises to which water service is being furnished, resides at or does business at a location other than the service address, and desires to have the water charges, notices and other communications sent to an address other than the service address, any such owner or authorized agent shall first submit in writing such request to the Water Department. Such request shall clearly state the date and nature of the, request, the effective date, the service address, the separate mailing address to which the water charges, notices and other communications are to be sent and the signature of the owner or authorized agent. The party submitting such request shall thereafter be responsible for providing written notice of any changes in the request until such time that he shall, in writing, withdraw the request. Thereupon the water charges, notices and other communications shall be sent to the premises to which water service is being furnished. Section 66: Monthly Charges - Dates Due and Payable. All water accounts shall be sent a statement of charges on a monthly basis. All water accounts served on a fixed rate basis shall be due and payable, in advance, not later than the 15th day after the date shown on the monthly statement, and if not paid within such time, shall be considered delinquent. A refund or credit will be given by the Water Department on fixed rate ser- vice charges paid in advance in the event of a vacancy, provided that notice in writing shall have been submitted to the Water Department at the time such -31-
