2 <br /> City of Everett <br /> REQUEST FOR PROPOSALS <br /> Proposal 2013-0XX <br /> DRAFT <br /> EMPLOYEE BENEFITS BROKER AND CONSULTING SERVICES <br /> Due Date: September XX, 2013 <br /> I. INTRODUCTION AND BACKGROUND <br /> A. Proposal Deadline <br /> Sealed proposals will be received by the City of Everett, City Clerk, 1st Floor, 2930 Wetmore <br /> Avenue, Everett, Washington 98201 until 2:00 p.m., September XX, 2013 for the provision <br /> employee benefits broker and consulting services. <br /> B. General Information <br /> The City of Everett is soliciting requests for proposals from qualified contractors to provide <br /> benefit broker and consulting services, including legal compliance, evaluation and ongoing <br /> actuarial analysis of plan financial experience, claims and future funding requirements as <br /> indicated in Section C, Description of Services/Scope of Work. The City will select the benefits <br /> broker and consulting firm whose proposal is deemed most advantageous to the City, and begin <br /> receiving services as soon as practical. <br /> The City of Everett is a first class charter municipal corporation that operates under an elected <br /> mayor and an elected seven-member City Council. The City's Labor Relations/Human <br /> Resources Department is responsible for administering employee health benefits. <br /> City Health Benefits Overview <br /> The City of Everett currently offers two medical plans: a fully insured Healthcare Maintenance <br /> Organization (HMO) and a self-insured Preferred Provider Organization plan (PPO). <br /> The City has offered the self-insured medical option to its eligible employees since 1994. <br /> Currently, approximately 73% of the benefit-eligible employees have selected the self-insured <br /> medical option, while 12.5% are enrolled in the City's HMO option (currently Group Health <br /> Cooperative). The Firefighters Union also has the ability to select coverage under the LEOFF <br /> Health and Welfare Trust. Healthcare Management Associates is the City's third-party <br /> administrator and PPO network provider for the self-insured medical plans. ING provides stop <br /> loss insurance for this plan. <br /> In addition to the medical benefits referenced above, the City of Everett offers: <br /> • Dental benefits through Washington Dental Service/Delta Dental <br /> • Vision benefits through HMA <br /> • Prescription drug benefits through CVS/Caremark <br /> • Health care and dependent care flexible spending accounts through Flex Plan Services <br /> • Group and voluntary life and AD&D insurance through The Standard <br /> 1 3 <br /> DRAFT 2 of 16 <br />
