MEMORANDUM OF UNDERSTANDING 2 <br /> 11. Provide technical support to the meal site relative to site operation(food service,reservations, <br /> donations,data collection). <br /> 12. Evaluate and monitor the nutrition services on a regular basis and provide feedback to the site <br /> director. <br /> 13. Close the meal program if the Everett School District is closed. <br /> 14. Reimburse the center for their Board of Health Permit up to a level B. If the center wishes to <br /> obtain a level C permit,they will be responsible to pay the difference. <br /> 15. Obtain a Release of Information form signed by the client before sharing any confidential <br /> information. <br /> 16. Homage will not,on the grounds of race,color,religion,sex,national origin,or handicap: <br /> a. Deny any individual any services or other benefits provided under the program <br /> b. Provide any services or other benefits to an individual which are different,or are provided in <br /> a different manner from those provided to others under the program <br /> c. Subject an individual to segregation or separate treatment in any manner related to his <br /> receipt of any service(s) or other benefits provided under this program <br /> d. Deny any individual an opportunity to participate in the program through the provision of <br /> services or otherwise,or will afford him an opportunity to do so which is different from that <br /> afforded others under the program. <br /> The City of Everett,the agency sponsoring the Nutrition Site agrees to provide the following: <br /> 1. Space and utilities for food preparation,service,clean-up,and storage that comply with <br /> Snohomish County Health District codes and regulations. For all activities using the kitchen <br /> facilities,assurance that all persons working in the kitchen possess a valid Food Worker Card and <br /> that health district codes and regulations,including HACCP(Hazard Analysis of Critical Control <br /> Points)are followed. <br /> 2. A disaster plan that includes procedures for meal service in the event of an emergency. <br /> 3. Serve menu as published using Homage recipes. Notify Homage BEFORE changing the recipes, <br /> menu and/or using donated foods. Approval must be granted by Homage before proceeding in <br /> these situations. <br /> 4. Communicate any changes in forecasted meal numbers one week in advance of the meal to allow <br /> time for Homage to adjust incoming deliveries of food and supplies. These changes can be for <br /> increases(such as classes or special meals)or decreases(such as a bus trip that results in <br /> lowered attendance). Homage is unable to guarantee that changes made less than a week in <br /> advance of the meal will be honored. <br /> 5. Meal site operation in accordance with the Senior Nutrition Services Program Standards of <br /> Washington State Department of Social and Health Services,Aging and Adult Services <br /> Administration and Homage Standard Operating Procedures <br /> https://www.dshs.wa.gov/sites/default/files/ALTSA/hcs/documents/SNPStandards.pdf . <br /> Responsibilities include: <br /> a. Collection of data pertinent to the Nutrition Program. Data to include complete meal sign-up <br /> sheets,participant registration forms,caterer's invoices,and meal counts. In lieu of specific <br /> Nutrition Program registration forms,the site will provide copies of their own completed <br /> center registration forms. <br /> b. Collection of all donations for meals in a locked safe provided by Homage. <br /> c. Collection of$7.50 per meal for all ineligible persons eating a meal. This includes everyone <br /> under the age of 60 unless they are a meal site volunteer,a spouse or an unpaid family <br />
