MEMORANDUM OF UNDERSTANDING 3 <br /> caregiver accompanying the eligible person to the site. All payments should be placed in <br /> provided envelopes and inserted into the locked safe. <br /> d. Post a sign next to the donation box that clearly states the requested donation and the fee for <br /> ineligible persons. <br /> e. Post a large print menu in an area that is easy for participants to see. <br /> f. Post menus in the kitchen and dining rooms. <br /> g. Post emergency phone numbers in the kitchen.Emergency contact numbers should include <br /> (but are no limited to)key Center staff including maintenance,key Homage staff,Snohomish <br /> Health District,poison control center,911. <br /> h. Post Homage grievance procedure(provided by Homage)in dining or other area easily seen <br /> by participants. <br /> 6. Notification to Homage as early as possible each day the site will be closed due to weather or <br /> other factors such as power outages. <br /> 7. Emergency contact information for critical staff posted in the kitchen area and provided to <br /> Homage. <br /> 8. Submit paperwork to Homage according to the following schedule. Paperwork must be available <br /> for the nutrition driver to pick up on the first delivery of each week: <br /> a. Weekly- <br /> i. Sign-in sheets <br /> ii. "Pink"sheet-plate count <br /> b. Monthly <br /> i. Refrigerator and freezer temperature records <br /> ii. Serving temperatures <br /> 9. Notification to Homage when a substitute cook(including volunteers)is working. <br /> 10. Notification to Homage thirty(30)days in advance of plans to cancel a regularly scheduled Title <br /> IIIC meal with a statement of how the participants will be fed. <br /> 11. Monitor food temperatures and assure safe food handling/storage to comply with HACCP <br /> guidelines. <br /> 12. Copies of annual reports of fire/safety and health inspections to the Nutrition Program of <br /> Homage. <br /> 13. Monitor and record temperatures of refrigerators,freezers,and dishwasher. Routine <br /> maintenance and timely repair of all kitchen equipment. In the case a piece of equipment is not <br /> properly functioning(such as oven,refrigerator,freezer,dish machine),Homage must be notified <br /> to assist in modification of menus as needed. Provide routine cleaning of kitchen,large <br /> equipment(ovens,hoods,refrigeration units,etc),and regular mopping of floors. <br /> 14. Referral of persons in need of nutrition counseling and home delivered meals to Homage <br /> Nutrition Program. <br /> 15. The sponsoring agency will not,on the grounds of race,color,religion,sex,national origin or <br /> handicap: <br /> a. Deny any individual any services or other benefits provided under the program <br /> b. Provide any services or other benefits to an individual which are different,or are provided in <br /> a different manner from those provided to others under the program <br /> c. Subject an individual to segregation or separate treatment in any manner related to his <br /> receipt of any service(s)or other benefits provided under this program <br /> d. Deny any individual an opportunity to participate in the program through the provision of <br /> services or otherwise,or will afford him an opportunity to do so which is different from that <br /> afforded others under the program. <br />
