4, <br /> EXIT A <br /> SCOPE OF WORK <br /> CITY OF EVERETT <br /> LAKE CHAPLAIN SOUTH DAM TUNNEL DECOMMISSIONING <br /> ENGINEERING REPORT AND DESIGN <br /> The City of Everett owns and operates the Lake Chaplain South Dam. The City <br /> contracted with Gray&Osborne to prepare an Engineering Predesign Report and Plans, <br /> Specifications, and Cost Estimates. The City now requires work to allow final design, <br /> permitting,bidding, and construction of the improvements recommended in the Predesign <br /> Report. This Scope of Work presents the professional services required for the project. <br /> The following tasks will be accomplished to develop the permitting and design <br /> modifications. <br /> Task 1—Project Management to Include Coordination of Subconsultant and <br /> Quality Assurance and Quality Control Reviews <br /> This item of work will incorporate the overall management of the work as well as <br /> coordination of subcontract work. In addition,the project management task,covers the <br /> administrative activities. <br /> Task 2—Permits and SEPA <br /> This work will include preparation of a State Environmental Policy Act(SEPA) <br /> checklist,JARPA submittal,and City permitting for the work area associated with the <br /> Tunnel Decommissioning project. The checklist shall be prepared for use by the City to <br /> assist in making a threshold determination regarding the proposed project. The <br /> preparation of an Environmental Impact Statement(EIS),if required, is not included in <br /> this scope. All costs for publication and circulation of the Notice of Determination of <br /> Non-Significance, as applicable,will be paid by the City. This task will also include <br /> contact and submission of the Engineering Report, 100 percent design Plans,and Joint <br /> Aquatic Resources Permit Application(JARPA)to the City, WDFW, and Department of <br /> Ecology for review and permit issuance. This task also include submittal to Ecology <br /> Dam Safety to address questions and comments regarding the project work,and <br /> complying with applicable Dam Safety standards. <br /> G&o 4'20145.83 Page 1 of 3 <br /> 16 <br />
