The intake process determines: <br /> 1)What are the participant's immediate barriers to success? <br /> 2)What is needed to assist each participant in being successful? <br /> 3)Which goals are necessary to achieve a successful life without being a repeat offender while fulfilling all court <br /> requirements? <br /> The Family Support Coach and Work Crew Manager create a tailored supportive service plan and program schedule <br /> for each participant.This plan and schedule shall be provided with the referral and tracking form submitted for each <br /> participant. Staff will utilize resources available through various social service providers in the area. <br /> Participants collect litter throughout the downtown core during program hours.Lunch is provided for each <br /> participant and the Work Crew Manager provides soft skill training during that time,including motivational videos <br /> and life skills instruction,based on the needs of current participants.At the end of each day,participants are <br /> evaluated to determine what they retained and what program adjustments would help participants maintain focus on <br /> their goals. <br /> Reporting: <br /> For each individual in the program a Court and Prosecutor Referral/Tracking Form and a Supportive Service Plan <br /> and Program Schedule will be submitted to the Courts and the Prosecutor upon completion of their time at Work <br /> Crew. <br /> A monthly summary report will be submitted that provide numbers of referred individuals,numbers of individuals <br /> that showed up,number of individuals that complied with service plan;number of days each individual attended. <br /> what services were referred(including litter clean up),what services referrals were completed. and the number of <br /> successful Work Crew completions. <br /> Page 13 <br /> (Form Approved by City Attorney's Office November 1,2009) <br />
