City of Everett 00 6800 - 3 <br /> 2020 Biosolids Removal UT3705-11 <br /> complete and submit the Monthly Apprentice Utilization Report. The report shall <br /> accompany the Contractor's Request for Progress Payment. A Request for <br /> ' Progress Payment that is not accompanied by the Monthly Apprenticeship <br /> Utilization Report will not be processed for payment until said Monthly <br /> ' Apprenticeship Utilization Report is received and approved by City. <br /> B. The Contractor shall be responsible for reporting apprentice utilization data <br /> required by City beginning with the first day of work for each apprentice. The <br /> Contractor's first submittals are due at the end of the first month after the Contract <br /> start date specified in the Notice to Proceed, and at monthly intervals thereafter <br /> as specified above. Subcontractor submittals are due at the end of the month <br /> ' after commencement of their work and monthly thereafter, as specified above. <br /> C. The Contractor shall report the following information on each apprentice: <br /> 1. Apprentice's Name <br /> 2. Home Zip Code <br /> ' 3. Employment Status: New Hire or Existing Staff <br /> 4. Trade/Craft <br /> 5. State Apprentice Registration I.D. Number <br /> 6. Program Sponsor and/or Hiring Source <br /> 7. Apprentice Progression Period or Percentage <br /> 8. Ethnicity/Gender <br /> 9. Labor hours for Reporting Period by Ethnicity/Gender <br /> D. The Contractor shall report the following information on the total number of <br /> apprentices: <br /> 1. Total labor hours and number of apprentice employees for reporting period <br /> by ethnicity/gender. <br /> 2. Total apprentice labor hours and number of employees to-date. <br /> 3. Summary information as noted on the form. <br /> ' E. The Contractor shall report the following information on journey level employees: <br /> 1. Labor hours for reporting period by ethnicity/gender, for each trade/craft. <br /> 2. Total journey level labor hours by ethnicity/gender, for each trade/craft. <br /> 3. Total labor hours and number of journey level employees for reporting <br /> period by ethnicity/gender. <br /> 4. Total journey level labor hours and number of employees to-date. <br /> 1111 5. Summary information as noted on the reporting form. <br /> F. The Contractor shall submit such other information as may be requested by City <br /> to verify compliance with the Contract. City reserves the right to add, delete, or <br /> change as necessary the information required by the Contractor on the Monthly <br /> Apprentice Utilization Report form. <br /> 00 6800 - 3 APPRENTICE UTILIZATION AND REPORTING <br />
