City of Everett 00 63 63-4 <br /> 3600 Smith Street Clean Up <br /> Bid Item #3— Disposal. Modify bid item #2 titled, "Disposal", by adding an additional 557.31 tons <br /> of Disposal, at the unit price of$78.16 per ton, resulting in a price increase of$43,559.35 to the <br /> Contract. <br /> The net result of modify the two above bid items results in a Contract price increase of$50,068.73. <br /> Schedule B—Fill portion of proposal. Change Order No. 1 modifies one item on the Fill portion <br /> of the proposal as follows: <br /> Bid Item #2— Hauling, Fill and Compaction — Gravel Borrow. Modify bid item #2 titled, "Hauling, <br /> Fill and Compaction — Gravel Borrow", by adding an additional 215.88 tons of Hauling, Fill and <br /> Compaction — Gravel Borrow, at the unit price of$27.24 per ton, resulting in a price increase of <br /> $5,880.57 to the Contract. <br /> New Schedule C. This new schedule created to add the cost of a 9.8%tax on 15%of the contract <br /> subtotal, per the Toxic Cleanup Guidance requirements the City of Everett received in e-mail <br /> dated October 15, 2019, by the Department of Ecology. This results in an additional cost of <br /> $5,887.13 to the Contract. <br /> New Schedule D. This schedule adds three (3) new items to the contract as follows: <br /> New item #1 — Sump Cleaning. Create new item titled Sump Cleaning, in the agreed upon lump <br /> sum price of$3,785.49. <br /> New item #2— Disposal of Concrete Sump. Create new item titled Disposal of Concrete Sump, <br /> in the agreed upon lump sum price of$4,154.68. <br /> New item #3— Placement of Quarry Spalls. Create new item titled Placement of Quarry Spalls, <br /> by adding 395.24 tons of Placement of Quarry Spalls, at the agreed unit price of $7.81 per ton, <br /> resulting in a price increase of$3,086.82 to the Contract. <br /> The net result of adding the above three (3) new items to the Contract results in a price increase <br /> of$11,026.99. <br /> The net result of modifications to items in schedules A& B, plus the adding of new Schedules C <br /> & D, results in a $72,863.42 Contract price increase. <br /> JUSTIFICATION: <br /> Schedules A & B. The three (3) each bid items modified in combined Schedules A & B above <br /> are the result of quantity overruns in the original projected quantities estimated in the bid proposal. <br /> 00 63 63 -4 CHANGE ORDER -AGREED <br />
