a. Within ten(10) days after the Effective Date of this Agreement, Employer <br /> shall deliver a payment in the total gross amount of One Hundred,Ninety-Two Thousand, and <br /> Five Hundred Dollars ($192,500.00)to Employee's Counsel of Record, payable to the Law <br /> Office of Thomas G. Jarrard, PLLC, for costs and attorney fees. Employer will issue form 1099s <br /> to Employee and Employee's Counsel of Record for this payment; <br /> b. Within ten(10) days after the Effective Date of this Agreement, Employer <br /> shall deliver,via direct deposit, a payment in the total gross amount of Twenty-Two Thousand, <br /> One Hundred, Thirty-Nine Dollars and Seventy Cents ($22,139.70)to Employee,which <br /> represents the pay-out value of Employee's unused sick pay and vacation. This payment will be <br /> subject to normal withholdings and Employer will include this amount on Employee's 2020 W- <br /> 2; <br /> c. On or before June 15, 2020,Employer shall deliver, via check mailed to <br /> Employee's home address, a payment to Employee in the total gross amount of Fifty-Two <br /> Thousand Dollars ($52,000.00), in settlement of Employee's disputed claims for non-economic <br /> damages. Employer will issue a Form 1099 to Employee for this payment; <br /> d. On or before June 15, 2020,Employer shall deliver,via direct deposit, a <br /> payment to Employee in the total gross amount of Eight-Eight Thousand Dollars($88,000.00). <br /> This payment will be subject to normal withholdings and Employer will include this amount on <br /> Employee's 2020 W-2; <br /> e. On or before January 10, 2021, Employer shall deliver, via direct deposit, <br /> a payment in the total gross amount of One Hundred Thousand Dollars ($100,000)to Employee. <br /> This payment will be subject to normal withholdings and Employer will include this amount on <br /> Employee's 2020 W-2; <br /> f. On or before,February 1, 2021, Employer shall deliver deliver,via direct <br /> deposit, a payment in the total gross amount of Forty-Five Thousand, Three Hundred, Sixty <br /> Dollars and Thirty Cents ($45,360.30) to Employee. This payment will be subject to normal <br /> withholdings and Employer will include this amount on Employee's 2020 W-2; <br /> g. On or before February 1, 2021,Employer shall deliver,via check mailed <br /> to Employee's home address, a payment to Employee in the total gross amount of Fifty <br /> Thousand Dollars ($50,000.00), in settlement of Employee's disputed claims for non-economic <br /> damages. Employer will issue a Form 1099 to Employee for this payment; <br /> h. Employer shall keep Employee on Employer's health plan through the <br /> earlier of May 31, 2021, or until such time as Employee obtains alternate health coverage. <br /> Employee's normal monthly contributions for health coverage will be paid by Employer. <br /> i.
