Exhibit "A" <br /> Scope of Services,Time of Performance, Budget, <br /> Payment, Notices, and Representatives <br /> I. Scope of Service <br /> A. Activities/Project Description <br /> The Subrecipient will be responsible for administering the Facility Restroom Improvements program <br /> in a manner satisfactory to the City and consistent with any standards required as a condition of <br /> providing these funds. Such program will include the following activities eligible under the Community <br /> Development Block Grant Program: <br /> Program Delivery <br /> Activity: <br /> The Facility Restroom Improvements will serve approximately 1,100 Person by providing public <br /> restrooms for the child abuse victims and families/guardians who come to Dawson Place for safety, <br /> justice, and healing. <br /> The scope of work for the Facility Restroom Improvements will consists of four public bathrooms <br /> located on two floors (2 on 1st floor and 2 on 2nd floor) in the main building at Dawson Place Child <br /> Advocacy Center. <br /> • To repair wall hung toilets (8 toilets; 3 urinals) whose structural supports have failed <br /> • To repair a failed vent coupling within the wall between the 2nd floor bathrooms. This vents <br /> gas from below street level to the roof. It has failed, and now vents gas/noxious odor into the <br /> 2nd floor bathrooms. <br /> • In conjunction with the above repairs, renew finishes on walls &floors, improve lighting if <br /> possible without major electrical work and renew mirrors and efficiency at sinks. <br /> • In 1st floor family bathroom, permanently remove partitions and urinal. Provide new wall-hung <br /> baby changing station. <br /> REMOVE <br /> 1. existing wall-hung toilets &failed structural supports <br /> 2. existing wall and floor tile <br /> 3. partitions, sinks, paper towel and soap dispensers, mirrors (preserve, protect, store off-site) <br /> RENEW: <br /> 1. Structural supports for wall mounted toilets (and urinals as needed) <br /> 2. Wall mounted toilets in 4 bathrooms and urinal on 2nd floor <br /> 3. 3 mirrors (to match the beveled mirror in 2nd floor men's room which will be reused) <br /> 4. Sinks (4), faucets (4), and associated plumbing, as necessary to present clean, efficient and <br /> economical appearance/use...(submittal requirement for owner approval) <br /> 5. Floor covering and improve slope/level as required <br />
