PUBLIC WORKS v. The geotechnical report prepared by Cobalt Geosciences includes soil logs for hand borings, but does not include a map of where the borings were conducted. Please provide a revised report which includes a map of the boring locations. Map provided to Erik Emerson. vi. Provide a revised geotechnical report or supplementary letter from the geotechnical engineer which addresses the effects of groundwater mounding on the design infiltration rates for all infiltration BMPs. Also address the probable route of groundwater flow and the potential impacts to adjacent properties due to the proposed infiltration BMPs. Refer to SSC-5 in Volume III, Section 3.3.7 of the SWMM for requirements. Addendum Letter attached. The following elements have been incorporated into the storm water/drainage design to minimize any adverse impacts on the adjacent property: 1. Overflows from the roof downspout infiltration trenches,roof downspouts and footing drains are all tight-lined down to the drainage system in Florida Dr. 2. The permeable asphalt access drive reservoir course has an overflow into the drainage system in Florida Dr. 3. A cutoff drain downslope from the roof downspout infiltration trench on Lot 5 has been added. This drain is also connect to the Florida Dr. system. vii. Please provide an Operations and Maintenance (O&M) manual as a standalone document,identified as the O&M manual for the project,which can be retained by the owner and in city records. The manual must contain the following information in a prominent location: Standalone 0& M Manual attached which addresses all the following items. • The party responsible maintenance and operation, • The requirement for the O&M Manual to be retained on or within reasonable access to the site, - • The requirement to transfer the Manual to any new property owner along with the property, and • The requirement that a log of maintenance activity be kept and made available for inspection by the City upon request that indicates what M&O actions were taken shall be kept. • Maintenance and operation information/requirements consistent with the provisions of Volume V of the Stormwater Management Manual for the BMPs and facilities present on the site. • Include maintenance procedures for catch basins, infiltration trenches. viii. On the first page of the O&M manual the 3rd paragraph and associated bullets appear to be applicable to an open pond. If so these should be deleted and/or replaced with language appropriate to the BMPs which will actually be constructed. Standalone 0 & M Manual revised deleting open pond. ix.
