The geotechnical report prepared by Cobalt Geosciences includes soil logs for <br /> hand borings, but does not include a map of where the borings were conducted. <br /> Please provide a revised report which includes a map of the boring locations. Map <br /> provided to Erik Emerson. <br /> vi. Provide a revised geotechnical report or supplementary letter from the <br /> geotechnical engineer which addresses the effects of groundwater mounding on <br /> the design infiltration rates for all infiltration BMPs. Also address the probable <br /> route of groundwater flow and the potential impacts to adjacent properties due to <br /> the proposed infiltration BMPs. Refer to SSC-5 in Volume III, Section 3.3.7 of the <br /> SWMM for requirements. Addendum Letter attached. The following elements <br /> have been incorporated into the storm water/drainage design to minimize any <br /> adverse impacts on the adjacent property: <br /> 1. Overflows from the roof downspout infiltration trenches,roof downspouts <br /> and footing drains are all tight-lined down to the drainage system in Florida <br /> Dr. <br /> 2. The permeable asphalt access drive reservoir course has an overflow into <br /> the drainage system in Florida Dr. <br /> 3. A cutoff drain downslope from the roof downspout infiltration trench on <br /> Lot 5 has been added. This drain is also connect to the Florida Dr. system. <br /> vii. Please provide an Operations and Maintenance (O&M) manual as a standalone <br /> document,identified as the O&M manual for the project,which can be retained by <br /> the owner and in city records. The manual must contain the following information <br /> in a prominent location: Standalone 0& M Manual attached which addresses <br /> all the following items. <br /> • The party responsible maintenance and operation, <br /> • The requirement for the O&M Manual to be retained on or within <br /> reasonable access to the site, - <br /> • The requirement to transfer the Manual to any new property owner along <br /> with the property, and <br /> • The requirement that a log of maintenance activity be kept and made <br /> available for inspection by the City upon request that indicates what M&O <br /> actions were taken shall be kept. <br /> • Maintenance and operation information/requirements consistent with the <br /> provisions of Volume V of the Stormwater Management Manual for the <br /> BMPs and facilities present on the site. <br /> • Include maintenance procedures for catch basins, infiltration trenches. <br /> viii. On the first page of the O&M manual the 3rd paragraph and associated bullets <br /> appear to be applicable to an open pond. If so these should be deleted and/or <br /> replaced with language appropriate to the BMPs which will actually be <br /> constructed. Standalone 0 & M Manual revised deleting open pond. <br /> ix.
