CITY OF EVERETT JOB ORDER CONTRACT <br /> WORK ORDER PROPOSAL - SCOPE OF WORK DETAIL <br /> PROJECT: Demolition - Evertrust <br /> Location: City of Everett November 10, 2017 <br /> 1611 24th Street Rev1: 12/07/2017 <br /> Everett, WA 98201 <br /> Estimated Duration: <br /> • 5 Days Subcontracting <br /> • 2 Weeks Preconstruction/Material Procurement from NTP <br /> o Includes 10-day wait period for abatement permit if/as required <br /> • 4-8 Days Construction <br /> • 5 Days Punchlist/Demobilization/Closeout <br /> Construction Start Date: <br /> • TBD <br /> Description: <br /> This project consists of full building demolition of an existing vacant residential dwelling and associated <br /> site remediation located at 1611 24th Street, Everett, WA. <br /> Contract Documents and/or General Information: <br /> The scope of work for the Demolition-Evertrust project is based on information gathered from the <br /> following: <br /> • Project Intake Form (11/07/2017) <br /> • Joint Scope Walk(11/13/2017) <br /> • Subcontractor Walk-Through (11/13/2017) <br /> • In conjunction with the below FORMA Construction scope of work detail <br /> General information <br /> • The performance of all work to be in accordance with OSHA and WISHA safety requirements <br /> • Hazardous material abatement and disposal to be in accordance will all local, state, and federal <br /> requirements <br /> • The performance of all work to be in accordance with applicable construction and buildings codes <br /> and industry standard best practices <br /> • All construction debris will be cleaned up during construction and disposed of at an approved off- <br /> site location unless otherwise specified <br /> • Final cleaning of the site to remove any remaining debris or materials shall be accomplished at <br /> the conclusion of the project <br /> • This is a prevailing wage project <br /> City of Everett Project Specifications—Prior to Beginning Work <br /> • Traffic Control Plan Approval <br /> o Traffic Control Plan must be submitted at least two weeks prior to work beginning and <br /> must be approved by the City Traffic Engineer <br /> • All utility locates to be complete prior to on site Pre-Construction meeting <br /> o Contractor(s)to contact the City of Everett for locating the existing underground irrigation <br /> lines in the planting strips <br /> FORMA CONSTRUCTION COMPANY " J <br /> 1016 1ST AVE SOUTH##400 SEATTLE WA 98134 206.626.0256 <br />
