WORK ORDER PROPOSAL-SCOPE OF WORK DETAIL <br /> CITY OF EVERETT JOB ORDER CONTRACT <br /> • On site Pre-Construction meeting with City to review scope of work, operations, occupant <br /> disruption mitigation plan(s), public safety plan, and traffic control plans as required and <br /> appropriate <br /> WORK PLAN <br /> • Abatement <br /> o Provide all labor, material, and equipment to complete all abatement work as per <br /> attached documents and inclusive of the following scope of work: <br /> o Investigate, test, assess and determine amount of hazardous waste <br /> o Provide any/all permits, sampling, testing, reports, and disposal methods required to be <br /> in full compliance with all local, state, and federal requirements <br /> • Site Protection <br /> o Provide all labor, material, and equipment to complete all site protection work as per <br /> attached documents and inclusive of the following scope of work: <br /> o Provide temporary construction fence to encompass entire Project Area prior to <br /> beginning demolition <br /> • Fenced area to include laydown and equipment storage areas <br /> • Temporary fencing to be removed at completion of project <br /> o Provide approved TESC as required by AHJ prior to beginning any work <br /> o Provide all required traffic control for the duration of the project as required by AHJ <br /> o Remove all site protection as required at completion of project <br /> • Building Demolition <br /> o Provide all labor, material, and equipment to complete all general building demolition <br /> work as per attached documents and inclusive of the following scope of work: <br /> o Provide water connection and/or hydrant permit if/as required during all building <br /> demolition efforts <br /> o Disconnect, cut, and cap existing above ground and underground utilities at property as <br /> required <br /> • Coordinate with the appropriate jurisdiction to permit and disconnect all utilities <br /> connected to the structure or site as it relates to the 1611 24th Street residential <br /> structure <br /> o Prep and brace the structure as needed to demolish the structure in the safest manner <br /> and to not disturb neighboring structures or property <br /> • It is assumed the structure has a basement in which all foundation walls will <br /> remain in place once the structure is removed, basement to be stripped of any <br /> finishes or other materials, and back-filled with approved material <br /> • Perforate remaining basement floor slab as required for drainage <br /> o Demolish the structure, sort through debris to dispose of appropriately. Includes demo <br /> and disposal of any personal debris left by the previous owner on the site to be disposed <br /> of appropriately <br /> • Site Remediation <br /> o Provide all labor, material, and equipment to complete all site remediation work as per <br /> attached documents and inclusive of the following scope of work: <br /> o Remove existing tree in its entirety at southwest corner of house <br /> o City of Everett to provide backfill material from owner-stock <br /> o Demolition subcontractor under this contract to provide rough grading, compaction, and <br /> final grading of imported backfill over perforate existing left-in-place basement floor slab <br /> to turn over a site free of holes, uneven surfaces, and ponding <br /> • Finished surface to align with existing adjacent finished surfaces within the <br /> boundary of this work <br /> FORMA CONSTRUCTION COMPANY <br /> 10161ST AVE SOUTH#400 SEATTLE WA 98134 206.626.0256 ii <br />
