Y <br /> I WORK ORDER PROPOSAL-SCOPE OF WORK DETAIL <br /> CITY OF EVERETT JOB ORDER CONTRACT <br /> Assumptions <br /> • Construction area to be cleared of occupants and all loose items prior to construction start <br /> • Any/All damage to existing-to-remain site conditions under the scope of this project to be repaired <br /> to as-new or better condition by subcontractor(s)at no additional cost <br /> • Protect the public at all times using a combination of warning signs, barricades, and company <br /> staff to prevent access to work zone(s) <br /> • Assumes that a"laydown area"for contractors and equipment will be within Project Area <br /> • Assumes no hazardous materials are to be uncovered, handled, or disposed of outside of this <br /> scope of work <br /> • Assumes existing soils are not contaminated <br /> • Assumes all utilities have been shut-off to structure scheduled to be demolished <br /> • Assumes building demolition will not undermine any adjacent structures or hard surfaces <br /> • Assumes no work to existing asphalt, concrete, or other adjacent finished site conditions <br /> • Assumes daily and final jobsite cleanup for all trades under this contract <br /> • Contractor and/or subcontractor to provide all required permits <br /> o Actual permit fees to be reconciled with City of Everett at project completion via <br /> Supplemental Work Order <br /> Exclusions <br /> • Excludes OT and off hours/weekend work hours <br /> • Excludes CAD as-built drawings <br /> • Excludes any scope not specifically stated above <br /> • Excludes special inspections <br /> • Excludes any/all code or regulatory compliance upgrades to existing electrical, mechanical, <br /> plumbing, fire protection systems, and/or any other existing building systems and/or site <br /> conditions not under this contract <br /> • Excludes project or construction delays due to other trades not under this contract <br /> • Excludes handling or disposal of any hazardous materials outside of the specific scope above <br /> • Excludes removing utility lines from foundation to street connection(s) <br /> • Excludes any site surveying or landscaping <br /> Safety <br /> • Site Specific Safety Plan <br /> • Approved vehicle and pedestrian traffic control plans as required <br /> Required Submittals <br /> • Traffic/Pedestrian control as required per AHJ <br /> Potential Disruptions to Occupants and Remediation Plan <br /> • Intermittent sidewalk and alley closures as required to maintain a safe work zone <br /> o Minimize any sidewalk closures due to high level of foot traffic <br /> Construction Staging Area <br /> • No Parking signage along north side of 24th Street in front of property;Appx(x3)parking spaces <br /> • Dumpster to be located in alley adjacent to property scheduled for demolition <br /> Hours of Operation <br /> • Normal work hours 7:00AM -5:00PM Monday-Friday <br /> Schedule Issues/Milestones <br /> • Target start date January 8, 2018 <br /> Hazardous Materials <br /> • No Good Faith Survey provided by City of Everett <br /> • Survey to be provided under this contract <br /> FORMA CONSTRUCTION COMPANY <br /> 1016 1ST AVE SOUTH#400 SEATTLE WA 98134 206.626.0256 3 <br />
