Y I WORK ORDER PROPOSAL-SCOPE OF WORK DETAIL CITY OF EVERETT JOB ORDER CONTRACT Assumptions • Construction area to be cleared of occupants and all loose items prior to construction start • Any/All damage to existing-to-remain site conditions under the scope of this project to be repaired to as-new or better condition by subcontractor(s)at no additional cost • Protect the public at all times using a combination of warning signs, barricades, and company staff to prevent access to work zone(s) • Assumes that a"laydown area"for contractors and equipment will be within Project Area • Assumes no hazardous materials are to be uncovered, handled, or disposed of outside of this scope of work • Assumes existing soils are not contaminated • Assumes all utilities have been shut-off to structure scheduled to be demolished • Assumes building demolition will not undermine any adjacent structures or hard surfaces • Assumes no work to existing asphalt, concrete, or other adjacent finished site conditions • Assumes daily and final jobsite cleanup for all trades under this contract • Contractor and/or subcontractor to provide all required permits o Actual permit fees to be reconciled with City of Everett at project completion via Supplemental Work Order Exclusions • Excludes OT and off hours/weekend work hours • Excludes CAD as-built drawings • Excludes any scope not specifically stated above • Excludes special inspections • Excludes any/all code or regulatory compliance upgrades to existing electrical, mechanical, plumbing, fire protection systems, and/or any other existing building systems and/or site conditions not under this contract • Excludes project or construction delays due to other trades not under this contract • Excludes handling or disposal of any hazardous materials outside of the specific scope above • Excludes removing utility lines from foundation to street connection(s) • Excludes any site surveying or landscaping Safety • Site Specific Safety Plan • Approved vehicle and pedestrian traffic control plans as required Required Submittals • Traffic/Pedestrian control as required per AHJ Potential Disruptions to Occupants and Remediation Plan • Intermittent sidewalk and alley closures as required to maintain a safe work zone o Minimize any sidewalk closures due to high level of foot traffic Construction Staging Area • No Parking signage along north side of 24th Street in front of property;Appx(x3)parking spaces • Dumpster to be located in alley adjacent to property scheduled for demolition Hours of Operation • Normal work hours 7:00AM -5:00PM Monday-Friday Schedule Issues/Milestones • Target start date January 8, 2018 Hazardous Materials • No Good Faith Survey provided by City of Everett • Survey to be provided under this contract FORMA CONSTRUCTION COMPANY 1016 1ST AVE SOUTH#400 SEATTLE WA 98134 206.626.0256 3
