ICITY OF EVERETT <br /> EVERETT Public Works <br /> WASHINGTON <br /> ADDENDUM NO. 1 <br /> I TO <br /> SEWER 0 UTILITY UPGRADES <br /> W.O. # UP-3691 <br /> MAY 12, 2020 <br /> NOTICE TO PLANHOLDERS: <br /> Addendum No. 1 contains the following revisions, additions, deletions and clarifications and is <br /> hereby made a part of the contract documents for the above named Project and shall be <br /> considered by all prospective bidders in submitting bids. <br /> Bidders must acknowledge receipt of Addendum No. 1 in the space provided on page SP-29 in <br /> Division P, Proposal Form. Failure to comply with this requirement may render the Bid non- <br /> responsive and may cause its rejection. <br /> This Addendum consists of 112 pages, including all revisions, and attachments. <br /> The Bid date, time and location for receipt of Bids are unchanged by this Addendum. <br /> THE CONTRACT DOCUMENTS ARE MODIFIED AS FOLLOWS: <br /> ' Bid Descriptions- <br /> Replace page SP-20. The following bid description has been added: <br /> Bid Item 104 and 105: Baseline COVID-19 Requirements <br /> Measurement and Payment: Lump Sum (LS) <br /> The lump sum bid for Baseline COVID-19 Requirements shall constitute complete compensation <br /> for all of Contractor's costs necessary for the duration of the Work for compliance with Baseline <br /> COVID-19 Requirements. <br /> PROPOSAL— <br /> Replace pages SP-21 through SP-28, per the attached. The following items have been added: <br /> Page SP-25 and SP-27: Lump sum bid items 104 and 105 for Baseline COVID-19 Requirements <br /> have been added to the Bid Schedule. <br /> DIVISION 1 - GENERAL REQUIREMENTS — <br /> Replace pages SP-45 through SP-140, per the attached.
