The bidder shall complete spaces on the proposal <br /> I 19 form that call for, but are not limited to, unit prices; extensions; summations; the total bid <br /> 20 amount; signatures; date; and, where applicable, retail sales taxes and acknowledgment <br /> 21 of addenda; the bidder's name, address, telephone number, and signature; the bidder's <br /> I 22 UDBE/DBE/M/WBE commitment, if applicable; a State of Washington Contractor's <br /> 23 Registration Number; and a Business License Number, if applicable.
