It will 17 also list estimated quantities, units of measurement, the items of work, and the materials 18 to be furnished at the unit bid prices. The bidder shall complete spaces on the proposal I 19 form that call for, but are not limited to, unit prices; extensions; summations; the total bid 20 amount; signatures; date; and, where applicable, retail sales taxes and acknowledgment 21 of addenda; the bidder's name, address, telephone number, and signature; the bidder's I 22 UDBE/DBE/M/WBE commitment, if applicable; a State of Washington Contractor's 23 Registration Number; and a Business License Number, if applicable. Bids shall be 24 completed by typing or shall be printed in ink by hand, preferably in black ink. The 25 required certifications are included as part of the Proposal Form.
