1 Intents and Affidavits 1 2 On forms provided by the Industrial Statistician of State L&I, the Contractor shall submit to 3 the Engineer the following for themselves and for each firm covered under RCW 39.12 I 4 that will or has provided Work and materials for the Contract: 5 6 1. The approved "Statement of Intent to Pay Prevailing Wages" State L&I's form 7 number F700-029-000. The Contracting Agency will make no payment under this 8 Contract until this statement has been approved by State L&I and reviewed by 9 the Engineer. 10 I 11 2. The approved "Affidavit of Prevailing Wages Paid", State L&I's form number 12 F700-007-000. The Contracting Agency will not grant Completion until all 13 approved Affidavit of Wages paid for the Contractor and all Subcontractors have I 14 been received by the Engineer. The Contracting Agency will not release to the 15 Contractor any funds retained under RCW 60.28.011 until "Affidavit of Prevailing 16 Wages Paid" forms have been approved by State L&I and all of the approved I 17 forms have been submitted to the Engineer for every firm that worked on the 18 Contract. 19 I 20 The Contractor is responsible for requesting these forms from State L&I and for paying 21 any fees required by State L&I. 22 23 Certified PayrollsIl 24 Certified payrolls are required to be submitted by the Contractor for themselves, all 25 Subcontractors and all lower tier subcontractors. The payrolls shall be submitted weekly 26 on all Federal-aid projects and no less than monthly on State funded projects. , 27 28 Penalties for Noncompliance 29 The Contractor is advised, if these payrolls are not supplied within the prescribed II 30 deadlines, any or all payments may be withheld until compliance is achieved.
