I <br /> COVID-19 Health and Safety Plan (CHSP) Inspection I <br /> The Contractor shall grant full and unrestricted access to the Engineer for CHSP <br /> Inspections. The Engineer (or designee) will conduct periodic compliance I <br /> inspections on the project site, staging areas, or yards to verify that any ongoing <br /> work activity is following the CHSP plan. If the Engineer becomes aware of a <br /> noncompliance incident either through a site inspection or other means, theIl <br /> Contractor will be notified immediately (within 1 hour). The Contractor shall <br /> immediately remedy the noncompliance incident or suspend all or part of the <br /> associated work activity. The Contractor shall satisfy the Engineer that the I <br /> noncompliance incident has been corrected before the suspension will end. <br /> • On page 55, the bulleted language $2,00,000 on line 32 shall be removed from I <br /> section 1-07.18(2) Additional Insured. <br /> • On page 64, the following shall be added to section 1-08.1 Subcontracting at line 22: I <br /> Supplement this section with the following: <br /> • On page 65, lines 34-40 of Section 1-08.4 Notice to Proceed and Prosecution of Work ' <br /> the following was changed to read: <br /> Notice to Proceed will be given after the contract has been executed and the I <br /> contract bond and evidence of insurance have been approved and filed by the <br /> Contracting Agency. The Contractor shall not commence with the work until the <br /> Notice to Proceed has been given by the Engineer. The Contractor shall commence <br /> construction activities on the project site within ten days of the Notice to Proceed <br /> Date, unless otherwise approved in writing. The Contractor shall diligently pursue <br /> the work to the physical completion date within the time specified in the contract. <br /> • On page 66, the following was added to section 1-08.4 Prosecution of Work at line 1: 111 <br /> 1-08.4.INST2.GR1 <br /> The last sentence of Section 1-08.4 is revised to read: <br /> 111 <br /> 1-08.4.OPT4.GR1 <br /> (April 6, 2020) <br /> No other work shall be performed on the site until the Contracting Agency has <br /> accepted the installation of high visibility fencing, as described in the Contract, and <br /> a COVID-19 Health and Safety Plan (CHSP) has been prepared in accordance with <br /> Section 1-07.4(2) as supplemented in these specifications, COVID-19 Health and <br /> Safety Plan (CHSP). <br /> • On page 94, lines 28 through 30 of Section 7-01.2 Materials shall be revised to read: I <br /> Heavy duty bridge scuppers, including additional grates for the City's stockpile, <br /> shall be East Jordan 5800Z Heavy Duty Bridge Scuppers with 5800M1 grate I <br /> manufactured by East Jordan, or an equivalent product approved by the Engineer: <br /> ims 1 <br /> AZ Page 4 of 5 <br /> 1 <br />
