I COVID-19 Health and Safety Plan (CHSP) Inspection I The Contractor shall grant full and unrestricted access to the Engineer for CHSP Inspections. The Engineer (or designee) will conduct periodic compliance I inspections on the project site, staging areas, or yards to verify that any ongoing work activity is following the CHSP plan. If the Engineer becomes aware of a noncompliance incident either through a site inspection or other means, theIl Contractor will be notified immediately (within 1 hour). The Contractor shall immediately remedy the noncompliance incident or suspend all or part of the associated work activity. The Contractor shall satisfy the Engineer that the I noncompliance incident has been corrected before the suspension will end. • On page 55, the bulleted language $2,00,000 on line 32 shall be removed from I section 1-07.18(2) Additional Insured. • On page 64, the following shall be added to section 1-08.1 Subcontracting at line 22: I Supplement this section with the following: • On page 65, lines 34-40 of Section 1-08.4 Notice to Proceed and Prosecution of Work ' the following was changed to read: Notice to Proceed will be given after the contract has been executed and the I contract bond and evidence of insurance have been approved and filed by the Contracting Agency. The Contractor shall not commence with the work until the Notice to Proceed has been given by the Engineer. The Contractor shall commence construction activities on the project site within ten days of the Notice to Proceed Date, unless otherwise approved in writing. The Contractor shall diligently pursue the work to the physical completion date within the time specified in the contract. • On page 66, the following was added to section 1-08.4 Prosecution of Work at line 1: 111 1-08.4.INST2.GR1 The last sentence of Section 1-08.4 is revised to read: 111 1-08.4.OPT4.GR1 (April 6, 2020) No other work shall be performed on the site until the Contracting Agency has accepted the installation of high visibility fencing, as described in the Contract, and a COVID-19 Health and Safety Plan (CHSP) has been prepared in accordance with Section 1-07.4(2) as supplemented in these specifications, COVID-19 Health and Safety Plan (CHSP). • On page 94, lines 28 through 30 of Section 7-01.2 Materials shall be revised to read: I Heavy duty bridge scuppers, including additional grates for the City's stockpile, shall be East Jordan 5800Z Heavy Duty Bridge Scuppers with 5800M1 grate I manufactured by East Jordan, or an equivalent product approved by the Engineer: ims 1 AZ Page 4 of 5 1
