WASTE MANAGEMENT LIMITATIONS OF SERVICE <br /> 1. Carrier will not knowingly accept shipments including containers, which are not properly packaged <br /> or identified. <br /> 2. Carrier may refuse to accept shipments not immediately available for pickup at designated pickup <br /> areas adjacent to a loading dock or otherwise immediately accessible to Waste Management of <br /> Washington vehicles. <br /> 3. Carrier may refuse to pick up materials from points where the designated pickup area is obstructed <br /> at the time of pickup. <br /> 4. Containers are included in pricing unless quoted separately <br /> 5. Carrier will need approved person to sign a manifest at the time of collection, prior to <br /> transport. <br /> Extra Charges: <br /> • In the event Carrier inadvertently accepts a container described in this paragraph, charges will <br /> be assessed depending upon associated cost for any cleanup or reverse transportation. <br /> • In the event of an overweight container a flat charge will be assessed in accordance with your <br /> service agreement. <br /> Container Weight Limits are as follows: <br /> Container Size Maximum Weight Limit <br /> 5-gallon white bucket 25 lbs <br /> 12-gallon white bucket 35 lbs <br /> 17-gallon Non Haz Reusable Tub 40 lbs <br /> 30-gallon Cardboard Box 50 lbs <br /> Other sizes for Hazardous/RCRA Vary by container size Weight/Size determined up actual time it is packaged <br /> b. Carrier shall not knowingly accept for transportation any shipment which does not meet packing, <br /> labeling and handling requirements imposed or required bylaw. <br /> I acknowledge that I have read and understand that the terms listed on these two pages are the accepted wastes <br /> and packaging/pickup requirements for my facility's Pharmaceutical Waste. I understand as the Generator of <br /> these wastes, that we are responsible for the proper packaging and segregation of this waste.
