Change Order Cover Sheet/For Internal City Use Only <br /> EXPLANATION/JUSTIFICATION <br /> Items Change Orders <br /> 1 $ 3,844.89 Relocate flagpole and install new light- Existing flagpole <br /> location posed a safety issue with athletes using the new <br /> surfacing between the fields. Flagpole was relocated and light <br /> installed for nighttime illumination of the US flag. <br /> 2 $ 6,588.45 Install ADA compliant concrete walkway from Sievers-Ducey <br /> sidewalk to Phil Johnson Ballfields Complex-A new crosswalk <br /> on Sievers-Ducey is proposed soon for people parking across <br /> the street from the Phil Johnson Ballfields Complex. The <br /> crosswalk location is different than the new concrete stairs <br /> location in the original design. Concrete stairs have been <br /> removed from the project and an ADA concrete walkway <br /> installed adjacent to where the new sidewalk is to be located. <br /> 3 $ 17,380.94 Purchase portable mounds for ballfields in place of"Big Brown <br /> Mounds" -The original design called for installation of"Big <br /> Brown Mounds" for adjustable pitching mounds. These <br /> mounds are no longer available. The City received a credit for <br /> the "Big Brown Mounds" in CO#001. Portable mounds were <br /> required in place of the "Big Brown Mounds" <br /> 4 $ 8,690.51 Modifications for installation of stormwater treatment bio-vault <br /> —The original design did not work for onsite conditions. The <br /> vault was resized to provide adequate loading capacity. <br /> Additional excavation and grading work was required to <br /> achieve appropriate elevations for site stormwater to enter and <br /> discharge from the new stormwater treatment bio-vault. <br /> 5 $ (13,527.95) Credit for removal of concrete steps from project-see item 2 <br /> "ADA Compliant Concrete walkway". <br /> 6 $ 1,747.17 Repairs to damaged e-layer by City personnel-City police K-9 <br /> unit was training at night on the property adjacent to Phil <br /> Johnson Ballfields and the K-9 and handler accidentally ran <br /> across freshly placed e-layer damaging a section. The e-layer is <br /> the safety cushioning beneath the turf. It had to be replaced. <br /> 7 $ 31,933.00 Soccer Goals for the complex. The fields where changed from <br /> baseball only to baseball and soccer. Everyone was focused on <br /> baseball equipment and the soccer goals were overlooked. <br />
