Educational Services Agreement <br /> Page 3 <br /> • Develop an application process to identify individuals who have been impacted by COVID-19. <br /> • Submit an end-of-calendar-year letter that outlines the projected v. actual expenditures associated with <br /> the program. <br /> • Bill City of Everett for$52,000 on October 15,2020. <br /> • Fully cooperate with the City of Everett so that the City may comply with funding and reporting <br /> requirements. <br /> City of Everett will: <br /> • Clearly outline and communicate all funding and reporting requirements and collaborate with EvCC to <br /> ensure its proposed program is eligible for funding and can meet all requirements before contracts are <br /> executed. <br /> • Pay all invoices within 30 days of receipt. <br /> COST <br /> IT Boot Camp(6-week program)for up to ten(10)participants(per cohort) X 1 = $ 52,000 <br /> Total Cost: $52,000 <br /> This competency based program assesses each student upon entry. Students are placed into the appropriate course <br /> and the cost associated is not adjusted based on program placement. <br /> Additional participants, above ten (10), will be an additional cost of$3,500 per participant with a maximum <br /> number of participants of twelve (12). Pricing includes all materials required to participate in the proposed <br /> training; including textbooks, student handouts, certification materials, and instruction. <br /> Additional sections of training reflecting the pricing included in this agreement can be contracted and agreed <br /> to by both parties in writing.
