Educational Services Agreement Page 3 • Develop an application process to identify individuals who have been impacted by COVID-19. • Submit an end-of-calendar-year letter that outlines the projected v. actual expenditures associated with the program. • Bill City of Everett for$52,000 on October 15,2020. • Fully cooperate with the City of Everett so that the City may comply with funding and reporting requirements. City of Everett will: • Clearly outline and communicate all funding and reporting requirements and collaborate with EvCC to ensure its proposed program is eligible for funding and can meet all requirements before contracts are executed. • Pay all invoices within 30 days of receipt. COST IT Boot Camp(6-week program)for up to ten(10)participants(per cohort) X 1 = $ 52,000 Total Cost: $52,000 This competency based program assesses each student upon entry. Students are placed into the appropriate course and the cost associated is not adjusted based on program placement. Additional participants, above ten (10), will be an additional cost of$3,500 per participant with a maximum number of participants of twelve (12). Pricing includes all materials required to participate in the proposed training; including textbooks, student handouts, certification materials, and instruction. Additional sections of training reflecting the pricing included in this agreement can be contracted and agreed to by both parties in writing. Pricing will remain valid until 12/30/2020.
