Exhibit "A" Scope of Services, Time of Performance, Budget, Payment, Notices, and Representatives I. Scope of Service A. Activities/Project Description The Subrecipient will be responsible for administering the Rental Assistance & Staffing program in a manner satisfactory to the City and consistent with any standards required as a condition of providing these funds. Such program will include the following activities eligible under the Community Development Block Grant Program: Program Delivery Activity: The Rental Assistance & Staffing will serve a total of 18 unduplicated City of Everett Households by providing rental assistance and utilities to each during this program year. General Administration [Description of general administrative services to be performed in support of activities noted above] Administrative services to be performed in support of activities noted above include outreach, eligibility screening and use of Language Link to ensure equity access. The Community Coordinator will connect with participants by phone to complete the screening and application, gather the necessary backup documentation and process the payment for the rent assistance and/or utilities as appropriate. The Landlord Engagement Specialist will contact the landlord to gather a copy of the lease, W9 and any pending utility bills information and back up information. The Fiscal Analyst will submit the invoices and report of activity. The Regional Director and Executive Director will assure program compliance. Everett residents that disclosed other needs not covered by this funding will be referred to other funding sources. B. National Objectives The Subrecipient certifies that the activities carried out with funds provided under this Agreement will meet the following CDBG National Objective, as identified by the City of Everett for priority in the 2019 Program Year, "Benefit low/moderate income persons" - as defined in 24 CFR Part 570.208. Appropriate Subcategory and Matrix Code under the National Objective: Limited Clientele 05Q: Subsistence Payments 24 CFR 570.207(b)(4) or 42 USC 5305(a)(8) + 24 CFR 570.482(c)(2) - One-time or short-term (no more than three months) emergency payments on behalf of individuals or families, generally for the purpose of preventing homelessness.
