We will meet the Limited Clientele subcategory by using processes in place utilized for the <br /> screening and record keeping of families applying for funding. YWCA will advertise the term of <br /> funds and eligibility criteria in various media networks. YWCA has been managing and accounting <br /> for Federal funding for 25 years and has over 30 contracts for services, including City of Everett <br /> Human Needs and HOME TBRA Program, ESG, CDBG; FEMA/EFSP; 5 HUD-COC contracts <br /> and DSHS. 100% of all programs are for low and moderate income persons/households. We have <br /> created a workflow and application process for all tenant based rental assistance programs by <br /> which participants' access, apply for and are screened to enter the program. All of these programs <br /> and funding sources have eligibility criteria specific to their programs. YWCA staff will screen <br /> households using the specific criteria for this CDBG CARES funding related to income limits, <br /> location and length of assistance. Rental assistance including past rent due, current rent due, and <br /> utility assistance will be distributed to each household based on an average of $2,500 per <br /> household. Staff will obtain the appropriate Release Of Information (ROI) consent to communicate <br /> with the landlord, obtain a copy of the lease as well as other back up documentation to verify the <br /> location, past rent/utilities and to assure it does not exceeds more than 3 months. Staff assigned <br /> to this contract will obtain and file all needed documentation <br /> C. Performance Monitoring <br /> The City will monitor the performance of the Subrecipient against goals and performance standards <br /> required herein through quarterly reports and site visits. Substandard performance as determined by <br /> the City will constitute non-compliance with this Agreement. If action to correct such substandard <br /> performance is not taken by the Subrecipient within a reasonable period of time, as determined by <br /> the City and after being notified by the City, contract suspension or termination procedures will be <br /> initiated. <br /> D. Levels of Accomplishment - Goals and Performance Measures <br /> In addition to the normal administrative services required as part of this Agreement, the Subrecipient <br /> agrees to provide the following levels of program services: <br /> Activity Units of Service* per Month Total Units/Year <br /> Rental assistance and 1 . 5 18 <br /> utilities <br /> Total People or Households to be served Total unduplicated Everett Residents or <br /> Households to be served <br /> 32 18 <br /> *Note: Provide definition of Units of Service, which may include such measures as units rehabbed, <br /> persons or households assisted, or meals served, here. If this is a construction project, please <br /> explain here how the improvements will be used and how they benefit the community, what the <br /> target population is for the project, etc. <br />
