DECLARATION <br /> City of Everett is hereby applying for STOP Grant <br /> Continuation/Modification Funding for FFY 2020 through the Department of Commerce to support <br /> a community response to victims of domestic violence, sexual assault, dating violence or stalking, <br /> age of victims 11 or older. By checking "Accept" on this Declaration,we agree to the following: <br /> Activities that Compromise Victim Safety <br /> We have reviewed the list of Activities That Compromise Victim Safety and Recovery and Undermine <br /> Offender Accountability below and certify activities will not jeopardize victim safety. We understand <br /> the list is not exhaustive and other activities may be prohibited based of the scope of our project. <br /> 1. Procedures or policies that exclude victims from receiving safe shelter, advocacy services, <br /> counseling, and other assistance based on their actual or perceived sex, age, immigration <br /> status, race, religion, sexual orientation,gender identity, mental health condition, physical <br /> health condition, criminal record,work in the sex industry, income or lack of income, or the <br /> age and/or sex of their children. <br /> 2. Procedures or policies that compromise the confidentiality of information and/or privacy of <br /> persons receiving services. <br /> 3. Procedures or policies that require victims to take certain actions (e.g., seek an order of <br /> protection, receive counseling, participate in couples counseling or mediation, report to law <br /> enforcement,seek civil or criminal remedies) in order to receive services. <br /> 4. Procedures or policies that fail to include conducting safety planning with victims. <br /> 5. Project designs, products, services, and/or budgets that fail to account for the unique needs <br /> of individuals with disabilities,with limited English proficiency, or who are Deaf or hard of <br /> hearing, including accessibility for such individuals. <br /> 6. Using technology without addressing implications for victim confidentiality,safety planning, <br /> and the need for informed consent. <br /> 7. Partnering with individuals or organizations that support/promote practices that compromise <br /> victim safety and recovery or undermine offender accountability. <br /> Coordinated Community Response (CCR) <br /> A representative from our organization will participate in mandatory CCR meetings as a member of the <br /> local or tribal CCR team.We agree to continue to connect and coordinate virtually, via teleconference, or <br /> in person at least quarterly. <br /> Consultation with Victim Services(if applicable) <br /> We have proposed new activities, have consulted with the members of our CCR team, and they are <br /> aware of changes from last year's funding cycle. A new consultation form will be provided. <br /> We understand that all terms and conditions in the original grant will still be applicable unless otherwise <br /> changed through an amendment process. <br /> By checking the "Accept" checkbox below, as the authorized or delegated representative of the <br /> agency,you are agreeing that the agency, and applicable sub recipients, will comply with the <br /> conditions in this amendment and original application.This proposal and any resulting amendment will <br /> be incorporated by reference into your STOP grant. <br /> ❑ ACCEPT <br />
