Please resubmit the following items to the Permit Services Counter with your resubmittal <br /> once all requested revisions have been made_ <br /> i. Transmittal Sheet: (1)available at the Permit Counter(and included herein for your <br /> use upon resubmittal). <br /> ii. Plans: (4) full size sets of revised plans, plus original City Markups. <br /> iii. Drainage Report: (2) copies of revised report. <br /> iv. SWPPP Report: (2) copies of revised SWPPP report. <br /> v. Easement Form: (1) copy of easement form for utility easement with legal <br /> .description and exhibit attached for review. Signing of this form must be notarized <br /> and can take place at the permit counter if needed, typically done at the time of <br /> permit issuance for convenience to the owner. <br /> vi. Cost Estimate for Permit Fee Calculation Form: (1) copy of cost estimate form <br /> filled out for-the City to use to calculate the Public Works Permit Fee. <br /> The Permit Counter is open Monday through Friday from 7:30am-12:0Opm and from <br /> 1:00pm-4:oopm(closed between 12:00pm-1:00pm for lunch). <br /> b.
