Exhibit A:Davis-Bacon(DB) Monitoring Procedures <br /> The following Monitoring Procedures will be undertaken by the COE: <br /> ❑ Upon receipt of wage determination: <br /> • A copy of the applicable wage determination <br /> • A HUD form 4010 <br /> ❑ Review completed Bid Specification for inclusion of DB rates and 4010 <br /> ❑ Receive notice of successful bidder from COE <br /> ❑ Check debarment of successful bidder prior to contract award <br /> ❑ Set up appointment with successful contractor to review Davis Bacon Act procedures(pre-construction <br /> conference) <br /> ❑ Conduct pre-construction conference to review DB and related requirements <br /> ❑ Ensure DB posters are properly displayed on site and list the Subrecipient and COE contacts <br /> ❑ Conduct weekly employee interviews <br /> ❑ Review submitted DB forms and Weekly Certified Payrolls for accuracy and completeness <br /> ❑ Compare"Employee Interview"forms with submitted payrolls <br /> ❑ Write Deficiency letters to prime contractor and any relevant subcontractors with copies to Owner and other <br /> funding provider(s)and follow up to ensure performance of the required corrective actions,including <br /> documentation of wage restitution. <br /> ❑ Review and approve submitted Draw requests for payment by Subrecipient(funds may be withheld to satisfy <br /> DB requirements if potential deficiencies are noted) <br /> ❑ Answer phone questions or emailed questions <br /> ❑ Upon completion of the Project and prior to release of any funds held for DB issues identified by the <br /> Subrecipient, make a final review for compliance with DB requirements by each of the contractors and <br /> subcontractors. Approve release of any funds withheld by COE. <br /> ❑ Take responsibility for locating unfound workers due wage restitution and Fed-wiring funds to HUD for <br /> unfound workers. <br /> ❑ Include the project in COE's Semi-annual Labor Standards Enforcement Report to HUD and submit <br /> individual contractor violation enforcement reports to HUD whenever a contractor's violations exceed <br /> $1,000.00 or are willful. <br /> 4 <br />
