Exhibit B:Section 3 Monitoring Procedures <br /> (when applicable) <br /> The following Monitoring Procedures will be undertaken by the Subrecipient: <br /> U When providing the Owner with the applicable wage determination, include the following HUD standard forms <br /> (these items need to be in the bid specification): <br /> • 51-Section 3 Goals and Results <br /> • S2-Section 3 Business Concern Representation <br /> • S3—Contractor Section 3 Opportunities Plan <br /> • S4—How will Section 3 impact awarding the contract <br /> • Cl—Section 3 Clause <br /> • C2—Contractor Section 3 Opportunities Plan and Reporting <br /> • Instructions for Completing Section 3 Plan and Reports <br /> ❑ Review completed Bid Specification <br /> ❑ Await"Notice to Proceed" <br /> ❑ Set up appointment with successful contractor for pre-construction conference to explain how Section 3 <br /> monitoring will be conducted <br /> ❑ In reviewing weekly certified payrolls,check to see if each contractor and subcontractor has completed and <br /> submitted a monthly Section 3 Employment Opportunities Plan Status Report,including utilization of Section 3 <br /> businesses <br /> ❑ If necessary,write Deficiency letters to prime contractor/subcontractor with copies to Agencies and other <br /> funding provider(s) informing them that Section 3 forms have not been received or that the forms do not <br /> show full compliance <br /> ❑ Review and approve submitted Draw request for payment by the Subrecipient(funds may be withheld to <br /> satisfy Section 3 requirements if potential deficiencies are noted) <br /> ❑ Answer telephone questions or emailed questions regarding Section 3 <br /> ❑ Upon completion of the project and prior to release of any funds held for Section 3 issues by the Subrecipient, <br /> make a final review for compliance with Section 3 requirements by each of the contractor/subcontractors. <br /> Approve release of any funds withheld. <br /> 5 <br />
