TO BE COMPLETED BY APPLICANT EVALUATION FOR <br /> AGENCY USE ONLY <br /> through the South Everett Forest Preserve from a large off-site wetland <br /> complex. Silver Lake Creek exits the Bruskrud Road Trail site via a culvert <br /> beneath a driveway to the Snohomish County District 1 Public Utility property. <br /> The site is part of the greater Lake Washington drainage basin and specifically <br /> within the North Creek sub-basin. <br /> 2)Will the project require any work over,in,or adjacent to(within 200 feet)the <br /> described <br /> waters?If yes,please describe and attach available plans. <br /> No surface waters will be impacted. An improvised boardwalk with wood <br /> pallets and boards exist where an informal trail crosses a wetland within the <br /> site. The project will avoid and restore continued wetland impacts at this <br /> location by providing a boardwalk wetland crossing at the same location where <br /> the public has created an existing impact by placing pallets and boards to walk <br /> on which restrict water flow in the wetland. Pallets and boards would be <br /> removed with the new boardwalk and the surface water connection of the <br /> wetland would be restored. Information about this feature is included in the <br /> Master Plan. This action is assumed to be self-mitigating since it will result in <br /> ecological benefit to the wetland. <br /> 3)Estimate the amount of fill and dredge material that would be placed in or removed <br /> from surface water or wetlands and indicate the area of the site that would be <br /> affected. <br /> Indicate the source of fill material. <br /> No fill will be placed in surface water or wetlands. Existing pallets and boards <br /> would be removed. <br /> 4)Will the proposal require surface water withdrawals or diversions?Give general <br /> description,purpose, and approximate quantities if known. <br /> No surface water withdrawals or diversions are required. <br /> 5)Does the proposal lie within a 100-year floodplain?If so,note location on the site <br /> plan. <br /> The project site is outside the 100-year floodplain. <br /> 6)Does the proposal involve any discharges of waste materials to surface waters?If so, <br /> describe the type of waste and anticipated volume of discharge. <br /> No discharges of waste materials will occur. <br /> b.
