TO BE COMPLETED BY APPLICANT EVALUATION FOR AGENCY USE ONLY through the South Everett Forest Preserve from a large off-site wetland complex. Silver Lake Creek exits the Bruskrud Road Trail site via a culvert beneath a driveway to the Snohomish County District 1 Public Utility property. The site is part of the greater Lake Washington drainage basin and specifically within the North Creek sub-basin. 2)Will the project require any work over,in,or adjacent to(within 200 feet)the described waters?If yes,please describe and attach available plans. No surface waters will be impacted. An improvised boardwalk with wood pallets and boards exist where an informal trail crosses a wetland within the site. The project will avoid and restore continued wetland impacts at this location by providing a boardwalk wetland crossing at the same location where the public has created an existing impact by placing pallets and boards to walk on which restrict water flow in the wetland. Pallets and boards would be removed with the new boardwalk and the surface water connection of the wetland would be restored. Information about this feature is included in the Master Plan. This action is assumed to be self-mitigating since it will result in ecological benefit to the wetland. 3)Estimate the amount of fill and dredge material that would be placed in or removed from surface water or wetlands and indicate the area of the site that would be affected. Indicate the source of fill material. No fill will be placed in surface water or wetlands. Existing pallets and boards would be removed. 4)Will the proposal require surface water withdrawals or diversions?Give general description,purpose, and approximate quantities if known. No surface water withdrawals or diversions are required. 5)Does the proposal lie within a 100-year floodplain?If so,note location on the site plan. The project site is outside the 100-year floodplain. 6)Does the proposal involve any discharges of waste materials to surface waters?If so, describe the type of waste and anticipated volume of discharge. No discharges of waste materials will occur. b. Ground: 1)Will ground water be withdrawn,or will water be discharged to ground water?Give general description,purpose,and approximate quantities if known. No groundwater will be affected.
