4. Risk, Performance, and Quality Assurance <br /> A. Provide one physical original and an electronic sample representation(s)of your work that would <br /> most resemble what the City of Everett would receive as final deliverables. The original must be <br /> provided with the RFP submittal,and the electronic copy must be received within twenty-hours <br /> after submissions are due to the following email address: bids@everettwa.gov. <br /> B. Submit no more than five (5) completed relevant project experiences,within the past ten <br /> years that demonstrate successful contract performance similar in size and scope as <br /> described in this RFP, include government experience. Include the following for each <br /> reference: <br /> a. Company name and full address <br /> b. Point of contact name,title,e-mail address,and phone number <br /> c.
