Lake Chaplain Road Culvert and Embankment Replacement <br /> Exhibit A, Page 3 <br /> ■ Land disturbance authorization issued by the City of Everett or Snohomish County. <br /> ■ Section 404/401 permit issued by the US Army Corps of Engineers(USACE). <br /> • Hydraulic Project Approval (HPA) issued by the Washington State Department of Fish and Wildlife. <br /> Anticipated Permitting Tasks: <br /> • GeoEngineers and City of Everett will interact with the permitting agencies to identify and discuss <br /> the necessary permitting requirements of each agency. This may include an initial site visit with <br /> Washington Department of Fish and Wildlife (WDFW) to determine the feasibility of the preferred <br /> alternative and requirements for the permits. <br /> ■ Complete a site visit to document and assess habitat and other relevant site conditions. This site <br /> visit will be performed by a GeoEngineers biologist that will complete the proposed permitting <br /> tasks. If necessary, the biologist will delineate wetlands observed in the immediate vicinity of the <br /> site. We estimate that the biologist will require one day in the field. We understand City staff will <br /> be available to accompany our biologist and assist in field measurements as necessary. <br /> • Prepare a Critical Areas Report for City Submittal to applicable local agencies and USACE. The <br /> Critical Areas Report will document the presence of areas regulated under Snohomish County <br /> Critical Area Code. These regulated areas include Woods Lake Creek, wetlands, erosion and <br /> landslide hazards and their associated buffer. As required by Snohomish County, our Critical Area <br /> report will present a plan to restore Critical Areas that are disturbed by the culvert replacement <br /> project. <br /> • Prepare documentation for a Specific Project Information Form. The City of Everett will submit to <br /> USACE. Based on our current understanding of the project, we assume that this culvert <br /> replacement may be eligible for a Programmatic Biological Evaluation of the project's potential <br /> effect on species and habitats listed under the Endangered Species Act. If not, additional scope <br /> will be required to develop application materials for an individual project permit. <br /> • Subcontract a Cultural Resource Study to assess the projects impacts to historic or culturally <br /> sensitive resources. This study is required by USACE. <br /> • City of Everett will prepare a SEPA checklist for the project. We assume that the City of Everett <br /> Public Works department will be the lead agency and the City will issue a Determination of Non- <br /> significance (DNS). <br /> • Prepare a Joint Aquatic Resources Permit Application (JARPA)and supporting documents including <br /> the 30% project drawings in 8-1/2 x 11 format. City of Everett will submit package to USACE. The <br /> JARPA will include written project narrative, vicinity map, site plan, a proposed stream diversion <br /> plan, fish removal and exclusion strategies and Temporary Erosion and Sedimentation Control <br /> (TESC)details. <br /> • City of Everett will submit an online HPA application to WDFW using their APPS system. The APPS <br /> submittal will include GeoEngineer's project details and basis of design, along with the SEPA <br /> determination of the City. <br /> • Prepare for a Snohomish County Land Disturbance Activity (LDA) Permit, along with support <br /> documents. <br /> GeoEngineers File No.0661-107-00 <br />
