Appropriate Subcategory and Matrix Code under the National Objective: Limited Clientele <br /> 05K: Tenant/Landlord Counseling 24 CFR 570.201(e) or 42 USC 5305(a)(8) + 24 CFR <br /> 570.482(c)(2) - Counseling to help prevent or settle disputes between tenants and landlords. <br /> Description of how the project will meet this subcategory: Snohomish County Legal Services <br /> determines eligibility of low-income residents by conducting an interview and completion of an in- <br /> take form which captures appropriate data of each prospective client. Once eligibility has been <br /> determined, SCLS works with clients to achieve program goals as listed in section 1A. SCLS keeps <br /> records of all clients served and will provide reporting as indicated in this agreement. <br /> C. Performance Monitoring <br /> The City will monitor the performance of the Subrecipientagainstgoals and performance <br /> standards required herein through quarterly reports and site visits. Substandard performance as <br /> determined by the City will constitute non-compliance with this Agreement. If action to correct such <br /> substandard performance is not taken by the Subrecipientwithin a reasonable period of time, as <br /> determined by the City and after being notified by the City, contract suspension ortermination <br /> procedures will be initiated. <br /> D. Levels of Accomplishment-Goals and Performance Measures <br /> In addition to the normal administrative services required as part of th is Agreement, the <br /> Su brecipient agrees to provide the following levels of program services: <br /> Activity Units of Service* per Month Total Units/Year <br /> Legal Assistance (HJP) 30 individuals <br /> to low-income residents 3 persons/households assisted during grant <br /> of Everett assisted per month term <br /> Total People or Households to be served Total unduplicated Everett Residents or <br /> Households to be served <br /> 300 persons/households to be served <br /> by the HJP (Housing Justice Project) 30 persons/individuals to be served <br /> countywide, 1/3 are residents of during the grant term <br /> Everett. <br /> *Note: Provide definition of Units of Service, which may include such measures as units <br /> rehabbed, persons or households assisted, or meals served, here. If this is a construction project, <br /> please explain here how the improvements will be used and how they benefit the community, what <br /> the target population is for the project, etc. <br />
