Description of how the project will meet this subcategory: SCLS determines eligibility of low-income residents by completing an intake form and conducting an interview. Once eligibility has been determined, SCLS will provide services to clients as outlined in the "Scope of Service" under section 1A. SCLS shall maintain records and complete reports as indicated in this agreement. C. Performance Monitoring The City will monitor the performance of the Subrecipient against goals and performance standards required herein through quarterly reports and site visits. Substandard performance as determined by the City will constitute non-compliance with this Agreement. If action to correct such substandard performance is not taken by the Subrecipient within a reasonable period of time, as determined by the City and after being notified by the City, contract suspension or termination procedures will be initiated. D. Levels of Accomplishment- Goals and Performance Measures In addition to the normal administrative services required as part of this Agreement, the Subrecipient agrees to provide the following levels of program services: Activity Units of Service* per Total Units/Year Month Legal Assistance 30 (HJP) to low- 3 persons/households individuals/households income residents assisted per month assisted during grant of Everett term Total People or Households to be Total unduplicated Everett Residents or served Households to be served 300 persons/households to be served by the HJP (Housing 30 persons/households to be served Justice Project) countywide, during this grant term 1/3 are residents of Everett *Note: Provide definition of Units of Service, which may include such measures as units rehabbed, persons or households assisted, or meals served, here. If this is a construction project, please explain here how the improvements will be used and how they benefit the community, what the target population is for the project, etc.
