CITY OF EVERETT SPECIAL PROVISIONS <br /> a. Have Industrial Insurance (workers' compensation) coverage for the bidder's <br /> employees working in Washington, as required in Title 51 ROW; <br /> b. Have a Washington Employment Security Department number, as required in Title 50 <br /> ROW; and <br /> c. Have a Washington Department of Revenue state excise tax registration number, as <br /> required in Title 82 RCW. ' <br /> (r) 4. Disqualification. Not be disqualified from bidding on any public works contract under <br /> RCW 39.06.010 or 39.12.065(3). <br /> (s) 5. Prevailing Wage Training. Unless Bidder has completed three or more public works <br /> projects and had a valid business license for three or more years, Bidder must have <br /> received Department of Labor and Industries training on the requirements related to public <br /> works and prevailing wage under RCW 39.12 and RCW 39.04. <br /> (t) 5. Certification of Wage Compliance. Within the three-year period immediately <br /> preceding the date of the bid solicitation, not have been determined by a final and binding <br /> citation and notice of assessment issued by the department of labor and industries or <br /> through a civil judgment entered by a court of limited or general jurisdiction to have willfully <br /> violated, as defined in RCW 49.48.082, any provision of chapter 49.46, 49.48, or 49.52 <br /> RCW. <br /> (u) 6. Apprentices. If the Project is subject to the apprenticeship utilization requirements <br /> in RCW 39.04.320, not have been found out of compliance by the Washington state <br /> apprenticeship and training council for working apprentices out of ratio, without <br /> appropriate supervision, or outside their approved work processes as outlined in their <br /> standards of apprenticeship under chapter 49.04 RCW for the one-year period <br /> immediately preceding the date of the bid solicitation. <br /> 1-02.1(2) Supplemental Bidder Responsibility Criteria <br /> (******) <br /> This Project will not be subject to supplemental bidder responsibility criteria. <br /> 1-02.2 Plans and Specifications , <br /> Delete all paragraphs in 1-02.2 and substitute the following: <br /> Information as to where Bid Documents can be obtained or reviewed will be found in the Call for <br /> Bids (Advertisement for Bids)for the Work. <br /> After Award of the Contract, the Contractor will receive up to six sets of the reduced Plans (11" x <br /> 17") and accompanying Special Provisions. In addition, the Owner will supply up to three sets of <br /> full size plans(22"x 34"). All Plans and Special Provisions will be conformed with addenda unless <br /> Contractor requests otherwise. <br /> Additional Plans and Special Provisions may be purchased by payment of the current printing <br /> costs. <br /> 1-02.4 Examination of Plans, Specifications, and Site of Work <br /> 1-02.4(1) General <br /> Delete the fifth paragraph of 1-02.4(1), beginning with "Bid prices shall reflect", and substitute the <br /> following: <br /> Bid prices shall include everything necessary for the completion of the Work including, but <br /> not limited to, providing the materials, equipment, tools, plant and other facilities, and the <br /> management, superintendence, labor, and all necessary testing services. <br /> Revise the first sentence in the paragraph that begins with "Any prospective Bidder desiring an <br /> explanation" to read as follows: <br /> SEI to SRI Intertie,SRO8 Rehab Division 1 —GENERAL REQUIREMENTS WO No.—UP3714 <br /> SP-44 <br /> I <br />
