Assessment forms will be based on similar forms created for projects previously <br /> collaborated on by Murraysmith and Industrial Systems. <br /> 3. Assessment site visits will be scheduled for(2) 1-week increments. Approximately <br /> half of the pump stations to be evaluated each of the 2-week total. <br /> 4. All sites will be accessible and qualified City personnel or an electrician will be able <br /> to open electrical equipment for viewing. <br /> 5. Control and instrumentation equipment will be evaluated for condition and life <br /> expectancy and for compliance with City standards. <br /> 6. Mechanical and HVAC equipment and controls will be evaluated by others. <br /> Exclusions: <br /> 1. Deliverables not defined herein. <br /> 2. Services not specifically listed herein <br /> 3. Site visits other than listed above. <br /> Fee Proposal: <br /> Industrial Systems, Inc. proposes to provide the services and materials listed above on a T&M <br /> basis. The fees shall be as listed in the summary below and include estimate for Travel & <br /> Expenses. <br /> Fee Estimate $43,160.00 <br /> Terms shall be as stated in the August 8, 2014, continuing services agreement between Murray <br /> Smith & Associates and Industrial Systems, Inc. The enclosed billing rates shall be considered <br /> as our standard rates.
