Project: WFP East Clearwell Roof Replacement <br /> Change Order No. 01 <br /> Change Order Effective Date: June 18, 2021 <br /> additional anchor bolts were installed. Item No. 35 compensates the Design-Builder for redesigning, <br /> fabricating, and installing the revised truss retention plates. <br /> Increases the Contract Amount by$7,028.09 <br /> Item No. 36: Repair Floor Joint Seals <br /> The condition of the of the floor joint seals could not be determined due to the collapsed roof. Once <br /> the Contractor removed the collapsed roof, it was found that approximately 100 lineal feet of the <br /> seals had been damaged. The seals were installed during the previous roof project and contributed to <br /> virtually eliminating leakage from the clearwell. The City's Design-Builder agreed that some of the <br /> seal damaged occurred during demolition of the existing roof. Because of this, it was agreed that the <br /> City would pay for 50 lineal feet of repairs. Item No. 36 compensates the Design-Builder for <br /> purchasing and repairing 50 lineal feet of floor joint seal. <br /> Increases the Contract Amount by$4,798.93 <br /> CONTRACT TIME: Change Order No. 1 adds 236 calendar days to the contract. <br /> Our Design-Builder's fabricator experienced Covid-19 delays during design,fabrication and delivery of <br /> engineering drawings and materials. <br /> Trucking firms were impacted by Covid-19. Due to a lack of drivers, delivery of materials was delayed. <br /> Due to a lack of fabrication personnel, fabrication errors were made. These errors were discovered on <br /> the project site. Additional time was required to fabricate and ship the replacement materials. <br /> Revising the truss retention slide plates added additional time.The revised slide plates had to be <br /> designed, the design reviewed by the City's Engineers, the plates had to be fabricated and shipped to <br /> the project site. <br /> Page 4 of 4 <br />
