October,2021 <br /> EXHIBIT"A" <br /> SCOPE OF WORK PERTEET INC. <br /> SILVER LAKE TRAIL SEPA AND SHORELINE PERMIT ASSISTANCE <br /> PROJECT UNDERSTANDING <br /> The Silver Lake Trail is being designed by the City to provide a hard-surfaced trail project around the perimeter of <br /> Silver Lake for recreational purposes.Design of the trail will be completed by others. <br /> The goal of the project is to permit the construction of ADA compliant trail surfaces through TA Sullivan park that <br /> will connect with Silver Lake Road on the North and Silver Lake Drive on the South. <br /> Everett Parks requested the assistance of Perteet to complete SEPA checklist and Shoreline permit applications. <br /> Task 1 —Project Management <br /> Perteet will provide project management of Perteet tasks under this scope along with project progress reporting <br /> and monthly invoicing for consulting time and expenses under this scope of services. <br /> Deliverables: <br /> • Project progress reports submitted with invoices <br /> Assumptions: <br /> • Project duration assumed to include up to three invoice periods. <br /> Task 2—Assistance for City to Prepare SEPA Checklist and for Shoreline Permit <br /> Application <br /> Perteet will assist the City of Everett to complete a SEPA checklist based on provided project information and <br /> readily available information of the environmental project context obtained from resource databases and GIS <br /> maps.A Shoreline Substantial Development Permit is also assumed which requires a pre-application meeting. <br /> Perteet will assist City to schedule and attend a virtual pre-application meeting with Planning and Community <br /> Development. <br /> Deliverables: <br /> • Provide notes from pre-application meeting.Meeting is assumed to be online with notes provided via <br /> email. <br /> • Complete draft and final SEPA environmental checklist for Everett Parks use and internal submittal to <br /> Planning and Community Development.SEPA checklist to be provided in MSWord file via email. <br /> • Assist to complete Land Use Application for Shoreline Substantial Development Permit,PDF document <br /> via email.Application to be signed by City. <br /> • Complete draft and final Joint Aquatic Resources Permit Application(JARPA)for Shoreline Substantial <br /> Development Permit.JARPA to be provided in MSWord file via email.JARPA to be signed by City. <br /> • Compile mailing list for City permit notice use,Excel file via email. <br /> 1 <br /> File location: Y:\Contract Administration\Contracts\Everett\20210234_Silver Lake Trail SEPA_10-6-2021\Everett Silver Lake Trail SEPA Scope of <br /> Services_September 2021.doc <br />
