DocuSign Envelope ID:2A1A1 B97-7E12-4D08-A7FA-F9FBEB801815 <br /> ATTACHMENT C— REPORTING <br /> The Contractor must provide quarterly written reports and host regular(monthly or quarterly) meetings with Commerce for <br /> project update purposes. Regular meetings should cover current status of the project and any barriers that may affect the <br /> project schedule. <br /> The Contractor shall issue a quarterly report to Commerce, no later than 15 days after the end of each quarter, describing <br /> the project activity that occurred during the quarter. The report form will be provided by Commerce. The intent is to collect <br /> a description of the project activity that occurred during the period, including but not limited to: <br /> 1. A narrative summarizing project activities, risks and issues mitigated, and lessons learned; <br /> 2. The project milestones met to date and anticipated in the subsequent quarter(such as through a project Gantt <br /> Chart schedule provided quarterly in Microsoft Project format showing actual progress to date along with the <br /> baseline schedule developed at project kickoff etc.); <br /> 3. Quarterly updated budget projections for project expenditures; <br /> 4. Any additional metrics required from the capital budget proviso, legislature, governor's office, or Commerce; <br /> Once construction is complete and the project is online, all EVSE data affiliated with EVSE status and its use will be <br /> available to Commerce upon request. Commerce reserves the right to specify the form and manner in which the data is <br /> provided. At a minimum, this must include: <br /> 1. Address where each EVSE is located; <br /> 2. Zip code of each EVSE; <br /> 3. Port type of each EVSE; <br /> 4. The number of unique charging events by month; <br /> 5. The average duration of each charging event by month; <br /> 6. The kilowatt hours delivered by each EVSE at each project site by month; and <br /> 7. Downtime at each EVSE at each project site by month where monitored. <br /> Upon Milestone A completion, the Contractor will submit an initial fact sheet. The Contractor will update and finalize this <br /> fact sheet prior to the submission of the final invoice. Commerce may request the fact sheet be updated between initial <br /> and final fact sheet as conditions warrant. <br /> If the capital asset(s) and/or property(ies)where the capital asset(s) is installed would be owned by an entity or entities <br /> other than the Grantee during the Time of Performance and/or the five (5) year minimum useful life of the capital asset, <br /> the following additional reporting requirements apply for the duration of the five (5) year minimum useful life: <br /> • Once per biennium: submit a current photograph of the capital asset(s). <br /> • Once per biennium: submit a certified statement that the capital asset(s) has been used for the grant purpose <br /> over the course of the intervening year, and reporting the following data to support this assertion: <br /> o The number of unique charging events for each EVSE by month; <br /> o The average duration of each charging event for each EVSE by month; <br /> o The kilowatt hours delivered by each EVSE at each project site by month; and <br /> o Downtime at each EVSE at each project site by month where monitored. <br /> Contract#«Contract_Number» 20 Electrification of Transportation Systems <br />
