Exhibit "A" <br /> Scope of Services, Time of Performance, Budget, <br /> Payment, Notices, and Representatives <br /> I. Scope of Service <br /> A. Activities/Project Description <br /> The Subrecipient will be responsible for administering the Independent Living - Housing program in a <br /> manner satisfactory to the City and consistent with any standards required as a condition of <br /> providing these funds. Such program will include the following activities eligible under the Community <br /> Development Block Grant Program: <br /> Program Delivery <br /> Activity: <br /> The Independent Living - Housing will serve 100 people and provide 175 units of service by <br /> providing systems navigation and emergency assistance to individuals with developmental <br /> disabilities and household members.. <br /> General Administration <br /> [Description of general administrative services to be performed in support of activities noted above] <br /> The Independent Living Program benefist individuals with intellectual and developmental <br /> disabilities (IDD) and their families living in the City of Everett. The goal of this project is to assess <br /> that our clients are appropriately connected to vital services by providing comprehensive support, <br /> information, resources, education, referrals and emergency assistance <br /> related to independence and stable housing in the community they live. The Arc is the only <br /> agency that has expertise in navigating these often complicated systems related to disability, and <br /> offering supports to individuals with I/DD. <br /> We will work with clients by scheduling personal appointments, phone conversation and workshops <br /> (currently happening via Zoom), as well as calls on behalf of the client, basic support linking clients <br /> to programs such as SSI, Washington Connection, Special Needs Trusts, etc. <br /> B. National Objectives <br /> The Subrecipient certifies that the activities carried out with funds provided under this Agreement will <br /> meet the following CDBG National Objective, as identified by the City of Everett for priority in the <br /> 2021 Program Year, "Benefit low/moderate income persons" - as defined in 24 CFR Part 570.208. <br /> Appropriate subcategory and matrix code under the CDBG National Objectives: <br /> Limited Clientele 05B Services for Persons with Disabilities 24 CFR 570.201(e) or 42 USC <br /> 5305(a)(8) + 24 CFR 570.482(c)(2) <br /> Description of how the project will meet this subcategory: The Arc mission is: The Arc fosters respect <br /> and access for individuals with intellectual and developmental disabilities and their families, giving them <br /> the power to achieve a full and satisfying life.
