support their individual needs: helping with emergency assistance, navigating future planning, and linking them to other community supports as needed. C. Performance Monitoring The City will monitor the performance of the Subrecipient against goals and performance standards required herein through quarterly reports and site visits. Substandard performance as determined by the City will constitute non-compliance with this Agreement. If action to correct such substandard performance is not taken by the Subrecipient within a reasonable period of time, as determined by the City and after being notified by the City, contract suspension or termination procedures will be initiated. D. Levels of Accomplishment - Goals and Performance Measures In addition to the normal administrative services required as part of this Agreement, the Subrecipient agrees to provide the following levels of program services: Activity Units of Service* per Month Total Units/Year support, information, resources, education, 15 175 referrals, emergency assistance, workshops Total People or Households to be served Total unduplicated Everett Residents or Households to be served 100 People 30 *Note: Provide definition of Units of Service, which may include such measures as units rehabbed, persons or households assisted, or meals served, here. If this is a construction project, please explain here how the improvements will be used and how they benefit the community, what the target population is for the project, etc. The Subrecipient agrees to track outcome performance measures as required by the U.S. Department of Housing and Urban Development (HUD) and the City. The Subrecipient will report the outcomes to the City on the following schedule Performance Period Report Due July 1, 2021 — December 31, 2021 February 28, 2022 January 1, 2022 — March 1, 2022 April 30, 2022 April 1, 2022 —June 30, 2022 July 31, 2022 Reporting forms must be in the format acceptable to the City of Everett. Exhibit "E" shall be used for all reporting requirements under this agreement.
