Exhibit "A" <br /> Scope of Services, Time of Performance, Budget, <br /> Payment, Notices, and Representatives <br /> I. Scope of Service <br /> A. Activities/Project Description <br /> The Subrecipient will be responsible for administering the Minor Home Repair program in a manner <br /> satisfactory to the City and consistent with any standards required as a condition of providing these <br /> funds. Such program will include the following activities eligible under the Community Development <br /> Block Grant Program: <br /> Program Delivery <br /> Activity: <br /> The Minor Home Repair will serve 95 Household by providing necessary health, safety, and <br /> accessibility related repairs to vulnerable adults that they cannot perform or afford, based on the <br /> General Administration. <br /> General Administration <br /> [Description of general administrative services to be performed in support of activities noted above] <br /> Houses and Mobile Homes on private property: A) Services provided by the Home Repair program <br /> may include plumbing repairs, furnance repairs or replacement, roof patching, window replacement, <br /> carpentry, grab bars, door locks, smoke alarms, accessibility modifications and other health and <br /> safety related home repairs. The Home Repair program allows a homeowner up to $5,000 for home <br /> repairs in a 5 year period. B)Home repair projects will be limited to $5,000 each of actual dollars <br /> spent (labor and materials) on single family homes on lots/property owned by the homeowner. Once <br /> this dollar amount has been reached, the client will be referred to the City of Everett's Community <br /> Housing Improvement Program (CHIP). This dollar amount calculation started with the July 2018 <br /> contract. This amount may be increased by the City after CHIP reviews the property to assess if the <br /> property/unit and client are eligible for CHIP funding. For clients who do not quailify for CHIP, with <br /> City of Everett approval, additional work may be done by Homage Home Repair up to $3,000, for a <br /> total of no more than $8,000 total project funds per property over a 5-year period. This amount may <br /> be increased with review and approval by the City of Everett after review of the safety needs of the <br /> home. Examples of eligible larger repairs may include but are not limited to: roof coating, tub to <br /> shower conversions, water heater or furnace replacements, ramps, and low-rise steps. <br /> Mobile Homes on leased/rented land: Services provided by the Home Repair program may include <br /> plumbing repairs, furnance repairs or replacement, roof patching, window replacement, carpentry, <br /> grab bars, door locks, smoke alarms, accessibility modifications and other health and safety related <br /> home repairs. The Home Repair program allows a homeowner up to $5,000 for home repairs in a 5 <br /> year period. With City of Everett Approval, additional work may be done by Homage. <br />
