CASINO RD, PERMIT#188766 11 July 2006 Dear Mr. Siddiq; In order for the City of Everett to properly assess our recent application for a SEPA review I would like to offer the following: CLEARING/GRADING PERMIT In June 10, 1996 Dariotis Developments Inc. applied for a clearing and grading permit. This application was determined to be complete by Don Wood on June 27, 1996.(e,A,A proposed mitigated determination-of non-significance, SEPA#34-96 was applied for,„ and a final mitigated determination of non-significance was issued on June 28, 1996.(re,E, A site plan was drawn up to obtain a public works permit, using a survey from Lovell- Sauerland&Associates, Inc. prepared on June 25, 1991,(refD,which showed the wetland area and the 25' buffer as accepted by Mr. Stephen Stanley, City of Everett, in a memorandum dated May 17, 1996.<«f.E,This site plan was approved by the city on September 8, 1996.(rcf.F,The site was posted and the affidavit of posting was notarized on July 3, 1996.(refG, A permit, #96-379,(,was issued on September 8, 1996. Work was commenced shortly thereafter,the site was cleared and graded and the work was completed according to the conditions on the permit. CONSTRUCTION TRAILER PERMIT On March 15, 1999 a permit was issued to Dariotis Construction Co. to place a construction trailer on the property at 903 W. Casino Rd.,permit#C61738.<«fI,This permit was based on a site plan which was based on the Lovell-Sauerland &Associates, Inc. survey from June 25, 1991. This site plan was approved on March 2, 1999��f n and shows the location of the construction trailer with a 20' side yard setback, and the 25' buffer around the wetland. l��
