CITY OF EVERETT SPECIAL PROVISIONS <br /> b) The required safety activities are being performed. <br /> c) The required safety equipment and facilities are up to date and maintained. <br /> d) Coordinate regularly with the City and/or the City's representative and providing them with <br /> requested documentation. <br /> i) The Engineer may, at his sole discretion, withhold payment should the Contractor not <br /> meet HASP and regulatory compliance, provide incomplete documentation, or show a <br /> disregard for health and safety. The Engineer may request additional documentation <br /> from the Contractor as needed to determine compliance with the HASP and regulatory <br /> requirements. <br /> 4. A total of 5% of the total contract amount will held for close-out activities and distributed upon <br /> completion of: <br /> a) Providing the City with requested contamination related, environmental, or safety and <br /> ' compliance documentation for project close out. <br /> b) Clean up, removal, and disposal of the project health and safety equipment and facilities. <br /> Should suspected contaminated or hazardous waste be encountered, "Material Handling and <br /> Disposal" items, such as payment for assisting the City representative to acquire confirmation <br /> samples, removal, stockpiling, and disposal or required remediation are addressed under <br /> other bid items and are not considered a part of this Work. <br /> I <br /> Bid Item 16 - Gravel Pad Construction <br /> Measurement and Payment: Square Yard (SY) <br /> The bid item includes all costs associated with furnishing the tools, labor, materials and equipment <br /> necessary to install and compact the gravel pad and geotextile layer . Removal of the existing <br /> gravel pad is addressed under Bid Item "Structure Excavation and Stockpiling, Incl. Haul. <br /> Bid Item 17 - Trench Cutoff Wall <br /> Measurement and Payment: Each (EA) <br /> The bid item includes all costs associated with furnishing the tools, labor, materials and equipment <br /> necessary to install a cutoff wall where indicated on the Plans. This item includes the cost of CDF <br /> necessary for construction. <br /> Bid Item 18 - Decommission Monitoring Well <br /> Measurement and Payment: Each (EA) <br /> The bid item includes all costs associated with furnishing the tools, labor, equipment, and materials <br /> necessary or incidental to decommissioning a groundwater monitoring well. This item includes <br /> coordination with Ecology and submittal of paperwork required for decommissioning. <br /> Bid Item 19 - Manhole 72-In. Diam. Type 3 <br /> ' Measurement and Payment: Each (EA) <br /> The bid item includes all costs associated with furnishing the tools, labor, equipment, and materials <br /> necessary or incidental to install a manhole, including frame and cover. This item includes <br /> ' pavement or concrete surfacing removal (if required), excavation, furnishing and final adjustment <br /> of the intake or cover frame or assembly, outlet traps, connection of the sewer pipe (including <br /> connector), channelization and controlled density fill, backfill and compaction of selected materials <br /> around the structure, cleaning structure, and restoration of adjacent areas in a <br /> POE Combined Sewer Main Imp.
