EXHIBIT A-1 <br /> WORK ORDER FORM <br /> DIKING IMPROVEMENT DISTRICT NO. 5 PERMITTING SUPPORT SERVICES <br /> AMENDMENT#1 <br /> SCOPE AND FEE ESTIMATE <br /> INTRODUCTION AND PROJECT UNDERSTANDING <br /> GeoEngineers, Inc. (GeoEngineers) is pleased to present this amended scope and fee estimate to the City <br /> of Everett(City)for additional permitting support services for Diking Improvement District No 5(DID5). <br /> Following preparation of the draft comment response memo developed under Task 3,a Mitigation Site Use <br /> Plan will be required for the DID5's Smith Island Estuary Restoration Advance Mitigation Site (AMS) to <br /> support permit applications for the District's Downstream Dike Maintenance work. <br /> The Snohomish County (County) collected vegetation data and photos during the summers of 2020 and <br /> 2021 but did not prepare a report for the AMS.The City requested Geoengineers review the County's 2020 <br /> and 2021 monitoring data and prepare a Year 3 monitoring report for DID5 AMS for submittal to the <br /> regulatory agencies. We have included an optional task for additional services related to monitoring and <br /> reporting to address unknown comments.The optional task(Task 7)will require written authorization from <br /> the City to be activated. <br /> SCOPE OF SERVICES <br /> Task 5-Smith Island Advance Mitigation Site Use Plan <br /> GeoEngineers will prepare a Mitigation Site Use Plan based on the requirements specified in the Smith <br /> Island Estuary Restoration Advance Mitigation Plan (AMP), As-Built and Year 1 Summary Memo and <br /> previously prepared permit applications and other project documentation for DIDS's proposed Downstream <br /> Dike Maintenance project. The Mitigation Site Use Plan will include the applicable mitigation ratio and <br /> number of credits to be applied based on above documents and coordination with City staff and applicable <br /> regulatory agencies. <br /> The Mitigation Use Plan requires a Mitigation Site Ledger as an attachment. GeoEngineers will coordinate <br /> with the City and Washington State Department of Ecology (Ecology) on selecting the appropriate ledger. <br /> GeoEngineers will complete the ledger based on the above documents and include as an attachment to <br /> the Mitigation Site Use Plan. <br /> Task 6-Smith Island Advance Mitigation Site Year 3 Monitoring Summary Report <br /> GeoEngineers will prepare a Year 3 Monitoring Summary Report for the DID5's AMS based on data collected <br /> by the County in 2020 and 2021.
