Limited Clientele; 05C, Legal Services 24 CFR 570.201(e) or 42 USC 5305(a)(8) + 24 CFR <br /> 570.482(c)(2) <br /> Description of how the project will meet this subcategory: SCLS only serves households with <br /> income levels at or below 200% of the Federal Poverty Level (FPL) (or at or below 80% of <br /> the Area Median Income-AMI), with the majority at or below 125% FPL (or 30% AMI). To <br /> determine whether an individual or family (household) is low- or moderate-income eligible <br /> for entry into our program, each prospective client completes an intake form containing <br /> questions regarding income amounts and sources of income and we compare that amount <br /> to the current income maximums based on family (household) size. To ensure ongoing <br /> qualification for our services, clients are required to update this data as their circumstances <br /> change or periodically as set by our internal controls processes. All data is tracked and <br /> report in our legal case management software. <br /> C. Performance Monitoring <br /> The City will monitor the performance of the Subrecipient against goals and performance standards <br /> required herein through quarterly reports and site visits. Substandard performance as determined by <br /> the City will constitute non-compliance with this Agreement. If action to correct such substandard <br /> performance is not taken by the Subrecipient within a reasonable period of time, as determined by <br /> the City and after being notified by the City, contract suspension or termination procedures will be <br /> initiated. <br /> D. Levels of Accomplishment - Goals and Performance Measures <br /> In addition to the normal administrative services required as part of this Agreement, the Subrecipient <br /> agrees to provide the following levels of program services: <br /> Activity Units of Service* per Month Total Units/Year <br /> Legal Assistance (HJP) 5 persons/households 60 persons-households <br /> to low-income residents assisted per month assisted during grant <br /> of Everett term <br /> Total People or Households to be served Total unduplicated Everett Residents or <br /> Households to be served <br /> 300 persons/households to be served <br /> by the Housing Justice Project (HJP) 60 persons/households to be served <br /> county-wide, 1/3 are residents of during the grant term <br /> Everett <br /> *Note: Provide definition of Units of Service, which may include such measures as units rehabbed, <br /> persons or households assisted, or meals served, here. If this is a construction project, please <br /> explain here how the improvements will be used and how they benefit the community, what the <br /> target population is for the project, etc. <br />
