1. Immediately, in no case more than 24 hours of becoming aware of the <br /> circumstances, notify Ecology of the failure to comply by calling the applicable <br /> regional office phone number (find at Ecology' Report a Spill webpage16). <br /> 2. Immediately take action to prevent the discharge/pollution, or otherwise stop or <br /> correct the noncompliance. <br /> 3. Submit a written report to Ecology using the WQWebPortal within five (5) days <br /> of the time the Permittee becomes aware of a reportable event. The report must <br /> contain: <br /> a. A description of the noncompliance and its cause <br /> b. The period of noncompliance including exact dates and times <br /> c. If the noncompliance has not been corrected, the anticipated time it is <br /> expected to continue <br /> d. Steps taken or planned to reduce, eliminate, and prevent reoccurrence of the <br /> noncompliance <br /> Ecology may waive the written report on a case-by-case basis upon request if the <br /> Permittee has submitted a timely oral report. <br /> Compliance with these requirements does not relieve the Permittee from responsibility <br /> to maintain continuous compliance with the terms and conditions of this permit or the <br /> resulting liability for failure to comply. Refer to Section G13 of this permit for specific <br /> information regarding non-compliance. <br /> H. ACCESS TO PLANS AND RECORDS <br /> 1. The Permittee must retain the following permit documentation (reports and <br /> monitoring records) on site, or within reasonable access to the site, for use by <br /> the operator or for on-site review by Ecology: <br /> a. Permit Coverage Letter <br /> b. Puget Sound Nutrient General Permit <br /> c. Discharge Monitoring Reports <br /> d. Attachments to the Annual or Single Report as required in the Nitrogen <br /> Optimization Plan (NOP) <br /> e. Nutrient Reduction Evaluation for Permittees listed in Tables 5 and 8 or <br /> AKART Analysis for Permittees listed in Table 11 <br /> S10. PERMIT FEES <br /> The Permittee must pay permit fees assessed by Ecology.
